Over or undermedicated on T3 mono?

Hi all,I need your help please.

I'm on T3 mono since September, gradually rising the dose until i reached 60/65 mcg day, splitted in 3 doses.

From then on, i have been feeling great for months, never felt so good in my life.

But about 3 weeks ago i started to feel absolutely sick: water retention, puffy eyes, dizziness, sense of faintness, hair hunger, brain fog and a crazy 120-130 heart rate.

I tried to rise the dose, and strangely i felt better for a coulpe of day, than things went even worse.

Since than i have been experimenting, and i found that the only way to keep my heart calm Is to stay at 30mcg T3 day, splitted in 5-6 microdoses.

On this dose my Heart stay at 95-100 during the day and goes around 80 in the night. 100 Is still a too high hr, but much better than 130.

Looking at my tracked values, i see that my HR was around 80 before taking T3, and It started to move up to 100 as soon as i reached 45mcg daily in november.

The problem Is that i think i'm hypo on this low dose, here you have my blood numbers (about 15 hours after the last dose)

Both ft4 and TSH are 0, my TSH went to 0 as soon as i take even a little amount of any medication, T3 or T4. My others blood value are all in good range (ferritin, d3, B12 and so on)

Before taking medication i didn't have any thyroid illness, i only had a little amount of haschimoto antibodies.

I have been treated because i had both ft3 and ft4 on the lower edge of the range, but my TSH was low-medium range.

I had all the classic hypo symptoms, and i have been treated especially because at that time i was suffering of a terrible gastroparesis, and my doctor tought It could be caused by my slow thyroid.

My gastroparesis in fact, desappeared when i reached 55-60mcg of T3 daily, but now that i'm taking only 30mcg my digestive system Is shutting down again, and i really don't want to return to a parenteral nutrition.

I have been treated with T4 mono: ft4 in good range, but ft3 never moved from the lower edge of the range, and i was even more sick, and TSH went to 0.

So i have started tò take T3 only.

I don't know how tò solve this situation, the questions in my mind that i hope someone here can help me:

-isn't weird to feel sick months after starting my treatment? Can my symptoms be caused by something else? My blood numbers don't looks to my like overmedication, at the opposite, the numbers on 30mcg looks like undermedication, but It's the only dosage that allow me to keep my symptoms under control

- Is It possible that i was feeling good because during the first months i still had some ft4 in my blood?

- could add some ft4 at my treatment be help full, or at the opposite, It could make the situation worse?

(P.s. when looking at my dosages, please keep in mind that i'm very small, i weight only 80 lbs, so maybe what's looks like a low dosage for normal people is an appropriate dosage for me.)

Someone much more knowledgeable will be along soon, but will need more details.

A timeline and results.

You started treatment because your free T3 and T4 were at the bottom of the range, when was this and what were the results?

You started on T4, dosage, for how long, what were your blood results, did you increase dose at all, if yes, what were your results on higher doses?

Did you try combination therapy, i.e. adding T3 to your T4 dose?

How long have you been taking T3 only, how quickly did you increase the dose to 70ug, the results above, are they on increasing dose, or decreasing dose, and how long did you leave it on each dose before testing?

I started tò take medication because my ft3 and ft4 were low, on the lower edge of the range. At the same time my TSH was low-medium. I had all the hypo symptoms, but nobody wanted to treat me because of my low TSH. My gastroenterologist suggest me tò start some kind of thyroid medication because my gastroparesis could be caused by my slow thyroid.

Two years ago i was treated with 75mcg T4 mono, my ft4 rose in a good range, but my ft3 didn't move at all. At the same time my TSH went to 0.

Than my doctor put me on T4+T3, but nothing change. I took something like 50 T4 + 15 mcg T3. So my doctor decided to stop tò give me medication.

Many months later, in September 2024 i decided to try T3 mono, especially because i could live with cold, hair loss etc, but i could not live with gastroparesis, so i decided to have a try.

My chronology Is:

- September last year i started T3, rising very slow my dose until reach 60-65 mcg on december. At this dosage i had an huge improvement in health, even my gastroparesis improved very much and i didn't need parenteral nutrition anymore.

- december - April: all ok, i was feeling the best in my life

- all of a sudden i start to feel absolutely sick, and i started to slowly reduce my dosage

All my blood test has been taken in this period, about 15 hours after the last dose. Before each test i stayed at least one week on that dosage

12/04/2024 70 mg

Ft3 5,82* (2,77 - 5,27)

26/04/2024 50 mg

Ft3 4,45 (2,77 - 5,27)

08/05/24 30 cmg

Ft3 3,97 (2,77 - 5,27)

Now i stay on 30mcg splitted in 5-6 doses, my Heart rate Is calmed a bit, but on this dosage i start to have hypo symptoms too: cold hands and feet, puffy eyes, water retention, very low blood pressure and i'm starting to see a mild form of gastroparesis
