My friends symptoms are overheating / burning up at night, they wake up as with dehydration to body and brain which then causes brain fog in the morning and a headache. like his head is shut off from his body and can’t function. He Can drink all day some days trying to make up for it and doesn’t recover. He says it’s his insides that burn up . He has had routine bloods done in past but with nothing abnormal detected .

What do you mean by routine blood tests? How long ago?
An unquenchable thirst can sometimes indicate diabetes - my father had that symptom 70 years ago plus the feeling of burning inside but the doctors put that down to some tropical diseases he'd got in the jungle in WW2.
Their b12 was 589 but 2021 . Because the doctor just told them to wear cotton pyjamas they won't go back as feel it is futile . They are male by the way so no menopause symptoms . Bit I did wonder if hormonal as male hormones
I have got the tsh and t4 results they had done now nearly 3 years ago . And I wondered do u think that by now they could be worse. Do u think levels like this would be of concern in USA just not in UK.
Diabetes and hypERthyrodism (over active thyroid) are both possibilities. Get blood glucose monitored. If he's had a thyroid blood test very recently, you can post the results here for opinions from members.
Thank you I am passing on all answers and hopefully they will go back to doctors and then will post results on here . I will also get them to join heath unlocked hopefully . As you guys always come up with answers for me so thank you
I'd be very cautious if he's drinking excessive amounts of water to relieve the burning sensation and quench his thirst. This can lead to hyponatraemia, or low sodium, as he could effectively flush it out. I know as it happened to me when I tried to do the same. Same reason, different cause but B12 related.

Happy to share experiences. I have no suggestions re diagnosis based on that alone but I would agree with Slowdragon that he should rule out B12 and other vitamin deficiencies as well as checking ferritin levels etc.
I identified with his symptoms and thirst but mine were to do with having B12 injections and having some reactions, which can happen. Update us when you have the test results.
