Hi I've struggled years with an underactive thyroid I have just received latest results wondered if anyone can help

Well thats just a TSH & FT4 and we do really need to see a full thyroid panel of TSH, FT4 & FT3 for a true picture of whats happening. All that said though and your TSH is far too high and FT4 too low although no reference range is shown.

What hormones are you taking currently?

What supplements are you taking?

What are your latest results for ferritin, folate, B12 & D3?

Are you on levo or NDT? Whatever you are on you need an increase!

If they are investigating pernicious anaemia I assume your B12 is low?

Low folate wouldnt indicate PA.

So how have your thyroid levels got so low and what are you taking?

I take 150 mcg thyroxine daily

So what has changed inbetween May when your levels although not optimal were much lower, and now?

How do you take your Levo? Its suggested to take on an empty stomach 1 hour away from food or caffeine containing drinks.

on replacement thyroid hormones we should be on high enough dose for TSH to always be below 2

Many (Most?) people when adequately treated will have TSH around or under 1

On just levothyroxine Ft4 at least over 60-70% through range

Ft3 at least 50-60% through range

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

On T3 or NDT - day before test split T3 as 2 or 3 smaller doses spread through the day with last dose 8-12 hours before test

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

I hope you’re receiving treatment I.e. replacement thyroxine as well for your hypo symptoms !, it is going to take a while to recover so you’ll have to be patient…wishing you the best..

On levothyroxine we must have GOOD vitamin levels

Many (most?) thyroid patients need to supplement vitamin D, magnesium, vitamin B complex

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and a week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

Low folate

This can help keep all B vitamins in balance and will help improve B12 levels too

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

Prescription from GP presumably?

Start separate B12 first ideally before adding folic acid

Once your folic acid prescription finishes, take daily vitamin B complex instead
