pills, pills & more pills.

Turns out, on top of adding four stones in weight since my Thyroid “treatment”(radioactive iodine), I’ve got gout. Clever chaps, these doctors. This wins me…yes you’ve got, folks…another, pat on the head from my own, personal Jesus aaaaaaand another prescription. Damn says I, I was hoping not to need yet another pill.

Anyone know if allopurinol will affect my thyroid function?

I’ve also (just about) run out of my Accord Levo and have been given Almus.

I know the wise ones hereabouts denounce changing brand.

Advice please & sorry for the silly stuff, but it is Saturday!!!

Paul

I don't know anything about allopurinal, because I didn't go to the doctor's when I had gout. But, there's a wealth of information in this post and the link it contains:

No apologies necessary. But, I don't speak Spanish, so I wouldn't know what you're talking about. lol

You have my sympathies, my dad had gout and it was very painful. Interestingly in the olden days gout was almost a status symbol as it was a rich person's disease, often associated with plentiful food, booze and a sedentary lifestyle.

The poor almost never got it as they couldn't afford luxuries. Not sure that was much consolation when they were suffering however. Hope you feel better soon.

Accord and Almus as the same brand. See helvella blogs.

How is gout even diagnosed?

Ask Dr Google! 🤣🤣🤣 Much more knowledgeable and informative than most doctors.

Look on the packet.

All packs have the PL Holder (or Manufacturing Authorisation Holder) on them which is visible without opening the pack. The position varies.

Almus has the same PL number and same PL Holder as Accord.

The blister packs inside are absolutely identical.

As are, of course, the tablets inside.

The UK document contains up-to-date versions of the Summary Matrix for tablets, oral solutions and liothyronine available in the UK.

great, thank you. I thought I’d read somewhere that a change of brand was a no, no.

helvellaAdministrator in reply toBigthensmallthenbig

It is usually best to stick to the same manufacturer - but that doesn't always mean the same name on the packet. As here.

And you sometimes have no choice.

And unless you have tried BBB you are unlikely to know if it suits you better than AAA!

Bloods. Raised uric acid. I asked for the test when I went for my last TFT’s because I was in moderate pain.

HealthStarDust profile image
HealthStarDust in reply toBigthensmallthenbig

So it’s a uric acid test? Is that right?

Bigthensmallthenbig profile image
Bigthensmallthenbig in reply toHealthStarDust

Yes. I just asked for a gout test. You know: mess around - find out. The needle was in anyway and my practice nurse requested it

previous post shows you are under medicated for thyroid and needed dose increase

healthunlocked.com/thyroidu...

Did you get increase to 175mcg daily….or even 150mcg and 175mcg alternate days

If not ….Push for dose increase ….gout is linked to low thyroid hormones

palomahealth.com/learn/gout....

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Test vitamin D, folate, B12 and ferritin

Stop any supplements that contain biotin 5-7 days before test

Bigthensmallthenbig profile image
Bigthensmallthenbig in reply toSlowDragon

No I’m still on 150. Feel a bit better. My last results were:

TSH: 0.72

T4: 14.8

T3: 4.7

The test was taken in accordance with your recommendations above. Although I’d dropped my levo dose the T3 had risen all be it, slightly.

This, to me indicated a conversion issue possibly so upping my dose of T4 wouldn’t help as T3 was actually lower at a much higher T4 dose. Do you think higher T4 might help the gout?

Thanks as always

pennyannie profile image
pennyannie in reply toBigthensmallthenbig

When T4 levels are too low for the individual the body will prioritise T3 over T4 so conversion can ' look ' good in that you have a ' better T3 ' but if there is not enough Free T4 circulating in the blood - you have no abilty to raise / convert T4 further to give you more energy as needed and likely feel you have hit a brick wall.

Does that make sense - just replied to your previous post - sorry I missed it - I'm with mild concussion and 2 weeks ago not in a good place -feel ok now - and make sense - I hope :

Oh no! Sorry to hear about the concussion. You need to stop skate boarding!! And yes perfect sense being made - thanks

I slipped backwards on a dollop of hair conditioner - out the bath while having a shower - landed on my back with my head hitting the open door edge :

My skateboard days never started - I couldn't even get on with roller skates and it took me over a year to learn to ride a bike - my father would take me round in a circle and looking back I think I spent more time sitting on his shoulder than anything else and just getting giddy.

Found memories - thanks for that :

SlowDragonAdministrator in reply toBigthensmallthenbig

FT4: 14.8 pmol/l (Range 7 - 18)

Ft4 70.91% through range

FT3: 4.7 pmol/l (Range 3.5 - 6.8)

Ft3 only 36.36% through range

So yes…..Poor conversion

when did you last test vitamin D?

NHS easy postal kit vitamin D test £31 via

vitamindtest.org.uk

20mcg vitamin D is only 800iu

Remember to stop vitamin B complex 5-7 days before test

So next step is test vitamin D, folate, B12 and ferritin before looking at getting T3 prescribed alongside levothyroxine

Sorry to hear about the gout. My brother has it & my Dad too. So painful. One wonders about genetics. I've no idea if there's a link to hypothyroidism. What I would say is that not being optimal on your thyroid hormones does mean your body does not run as efficiently as it could so may not be helping. Defo treat the gout. So painful and an attack is best caught early is my brothers understanding. Who knows maybe as you get your thyroid levels sorted the attacks may diminish or even disappear.... Be interesting to know.

Hope you feel better soon.

Have you had a sleep study done yet?

Please look up the connection between gout and sleep apnea.

You may be able to lose weight when you've got the APAP

(on demand) or CPAP (continuous flow).
