Test results - is T3 a bit too low?

Hello all,

I have received my blood test results and would appreciate some help. They are as follows:

TSH: 3.24 (range 0.27 - 4.2 nmol/L)

Free T3: 4 (range 3.1 - 6.8 nmol/L)

Free Thyroxine: 18.6 (range 12 - 22 nmol/L)

Vit D: 37 (50 - 250 nmol/L)

Everything else was OK apart from:

Thyroid peroxidase antibodies 177.2 (range 0 - 34 kIU/L)

Inflammation was also too high

The report highlighted Vit D insufficiency so I have ordered some to take care of that. However, it said that TSH, T3 and thyroxine were all normal and therefore I am on the correct dose of levothyroxine (currently 200 mcg).

I think TSH seems a bit high as in the past I've always felt not OK at anything above 2, also I was wondering if T3 seems a bit low within the range. (I also feel pretty awful). I would appreciate any help or insight into this.

What are you currently taking

Still just 200mcg levothyroxine or have you started T3 as well?

was test done early morning and last dose levothyroxine 24 hours before test

FT4: 18.6 pmol/l (Range 12 - 22)

Ft4 only 66.00% through range

FT3: 4 pmol/l (Range 3.1 - 6.8)

Ft3 very low at only 24.32% through range

Shows poor conversion and room for dose increase in levothyroxine

Vitamin D is far too low

Aiming for at least 80nmol minimum

How much vitamin D were you taking

Dose needs increasing and retest in 2-3 months

Please add B12, folate and ferritin results and ranges

High CRP (inflammation) can falsely raise ferritin levels

High thyroid antibodies confirms autoimmune thyroid disease also called Hashimoto’s

Are you on strictly gluten free diet

If not get coeliac blood test BEFORE considering trial on gluten free

Thanks very much for your reply.

I am on 200mcg currently and have not started T3 yet. I thought the results showed poor conversion but was confused by the report saying that all the thyroid levels were OK.

The test was done early morning and the last levothyroxine dose was just over 24 hours before the test.

I haven't been taking any Vitamin D (but will be soon).

Other results:

B12: 106 pmol/L (range 37.5 - 188 )

Folate: >45 nmol/L (range 8.83 - 60.8)

Ferritin: 81 ug/L (range 30 - 150)

CRP HS: 4.87 mg/L (range < 3)

I'm not on a gluten free diet, but have had a coeliac blood test before, which came back negative.

So you need to improve vitamin D

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function.

There’s a version made that also contains vitamin K2 Mk7. 

One spray = 1000iu

Suggest you start on 2000iu or 3000iu daily

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Test twice yearly via NHS private testing service when supplementing

You could try increasing levothyroxine by perhaps adding 12.5mcg daily

Or

try adding T3

Do you have 20mcg or 25mcg tablets ?

With either - cut into 1/4’s using sharp craft scalpel

Initially starting on just 1/4 tablet T3 in morning waking

Assuming ok …..after week add 2nd 1/4 tablet mid - late afternoon

Hold at this dose for 6-8 weeks then retest

Day before test last 1/4 tablet Approx 8-12 hours before test
