Help with dosage please.

Taking 100mcg levo, 25mcg Lio, (lio split into 2 doses). I have been on this dose for around 5 months.

Have included last medichecks results, first one in May, was full thyroid panel, 2nd only did basic thyroid test.

Although I still feel better than on T4 alone, I've not been feeling great recently, fatigue is getting bad, muscle and joint pain increasing, skin itchy, healing badly.

Any advice on next steps would be appreciated.

looking at previous post

Before adding T3 you were on 200mcg levothyroxine

You have reduced levothyroxine far too much dropping to 100mcg

We usually only reduce levothyroxine by 25mcg or maximum of 50mcg

Adding T3 invariably reduces Ft4 anyway

Most people when adequately treated will have Ft4 and Ft3 at least approx 50-60% through range

Increase levothyroxine by 25mcg

(cut 100mcg into 1/4’s using sharp craft scalpel)

Use a weekly pill dispenser to keep the other 1/4’s for following days

Retest in 6-8 weeks

Test early Monday or Tuesday morning, ideally before 9am

Last dose levothyroxine 24 hours before test

Last 1/2 or 1/3rd I’d daily dose T3 approx 8-12 hours before test

When were folate, B12 and ferritin levels last tested

Exactly what vitamin supplements are you currently taking

Vitamin D was slightly low on last test

Recommend getting vitamins tested too next test

In future

As SlowDragon has noted, you reduced your Levothyroxine too much prior to introducing Liothyronine. I would definitely add 25mcg Levo back. I am sensitive to change, so need to add increments of 12.5mcg only (using a pill cutter) & build up slowly. When you have added the full 25mcg for 6-8 weeks I would then retest.

Folate and ferritin were a bit low

Exactly what vitamin supplements are you currently taking

Have you increased iron rich foods in diet to improve low ferritin

Thank you both, will add more T4, I have a cutter it's what I used and still use to cut my Lio. Did start on quarter of a 25mcg, cutting the Levo down by 25 each time I added another quarter. Started Lio at beginning of this year.

Always retest 6-8 weeks after any dose change

Test early Monday or Tuesday morning ….watch out for postal strikes at moment
