Hello

Hope everyone is OK.

Please may I have advice on the following.

May

tsh 4.71 range 2.7-4.20

t4 12. 5 range 10-21

I then fought to get an uplift in levo see previous posts. I am currently on 100 and 75 on alternative days. I had my bloods tested at 3.30pm after taking 100 levo was at doctors for other reasons and they took them then .

today's results

tsh 1.43

t4 19.8 same ranges and 94% through range

I have recently been feeling as if I am on a rollarcoaster, have woken up with anxiety during the early hours, have also had a couple of episodes of palpitations.

Do you think that I should reduce my levo down to 87.5 per day instead of alternative days of 100 then 75. My tsh is the lowest its ever been not sure about t4 as they only test that now and again. Just want to feel well.

TSH would be higher if tested early in morning

ALWAYS test thyroid levels early morning, ideally just before 9am and last dose levothyroxine 24 hours before test

If on alternate different doses …..test 24 hours after LOWER dose

Just testing TSH and Ft4 is completely inadequate

When were vitamin levels last tested

What vitamin supplements are you currently taking

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested.

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease (Hashimoto’s or Ord’s thyroiditis)

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended on here that all thyroid blood tests early morning, ideally just before 9am and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

Hi thanks for answering. I always get my thyroid tested early on morning and stop levo until I have had test. However on this occasion I was at doctors for a different reason and she noted they were due and said she'd do them then. It was a totally unplanned thyroid blood test She did say that she would request t3 but surprise surprise it was not done. I just wondered as I was feeling the symptoms and my t4 was 94% through range I might benefit from a slightly lower dose ? Full bloods have been done weekly lately due to having chemo so as far as I'm aware everything is OK.

Always …if such an occasion arises again….say something like

“ Oh no….I am taking biotin….will have to stop for a week before test otherwise results will be unreliable …..I’ll book myself in next week”

You could try splitting your levothyroxine

Taking half daily dose waking and half at bedtime.

Day before test, take whole daily dose waking

Do blood test 24 hours later

Thanks that is another option. I have tried taking it on a night but didn't find any benefit so went back to morning.
