4/23/2022 120mg NP Thyroid:

Free T4 1.23 ng/dL 0.82-1.77

Free T3 3.1 pg/mL 2.0-4.4

7/23/2022 60mg NP Thyroid and 10mcg Liothyronine

TSH 0.817ulU/mL 0.450-4.500

Thyroxine (T4) 2.7 ug/dL 4.5-12.0

T3 Uptake 23% 24-39

Free Thyroxine Index 0.6 1.2-4.9

Free T4 0.48 ng/dL 0.82-1.77

Free T3 2.3 pg/mL 2.0-4.4

After my visit on 7/23/2022, my Dr. kept my NP Thyroid at 60mg and increased Liothyronine to 25mcg. I took this dosage for 2 days and each day I was extremely sleepy, overslept in the morning, face was puffy, blood pressure was lower, heart rate the same. Any idea why T3 would make me sleepy?

Increases in lio are recommended to be 5mcg every 2 weeks until dose is reached. This gives the body time to adapt. It is likely that you have increased too quickly.Your ft4 last result is under range Some folks manage with low ft4 on combo, ndt but others need it at 50%through range or higher. Increasing lio will make your ft4 even lower so this may be causing symptoms.

It took me 2 years of slowly adjusting levo and t3 doses 1 thing at a time with regular blood tests until I found the doses and dosing times that suited me. It is very individual and done by trial and error.

Taking T3 tends to lower FT4. Your FT4 on 7/23 was already below range. Increasing your T3 has very likely lowered it even more. This could be the reason. I take combination thyroid hormone replacement (Levo plus T3) and your result on 7/23 would have me bedbound, I need both around 60-70 % through range.

Your T3 should not have been increased from 10 to 25mcg in one go, we should increase by just 5mcg at a time, wait 6 weeks or so and retest plus see how we feel. Add another 5mcg of necessary, retest, etc, as before.

Adjusting dose this way means that we don't overshoot our sweet spot. Low and slow is the way to go.

You could try experimenting with the time of day you take your thyroid hormones. It isn't always necessary to change the dose if things aren't working well. Or you could split dose of one or both thyroid hormones.

Hi Kowbie. I research this side effect and found others have the same reaction. I also noticed my face puffy ( like when one is hypo). I called my dr and ask to go back to 120 mg NP thyroid because I felt better when I was on that dosage. I think he was trying to help my weight by adding in T3.
