Hi,

Is a TSH Level of 0.17 low enough to cause any symptoms?

We need to know the ranges your lab used to be able to comment on it

Please add ranges. Was anything else tested? You need to know FT3.

Do you currently take antithyroid or replacement?

Waterhouse,

Having glanced at your previous posts I see you were treated for an overactive thyroid with Carbimazole.

You will attract more relies relating to your condition if you supplied more info regarding present meds and anti-body levels if known, because usually in hypothyroidism (which you originally suffered) a low TSH is indicative of adequate thyroid hormone.

Members will also need test ranges to comment on T4 levels (numbers in brackets) and confirmation this result applies to free T4 (FT4).

How is your eye problem? Do you feel under or over (medicated)?

TSH range is 0.30-5.50

Free T4 range is 11.50-41

Free T3 level on letter from consultant is 5.7

I don’t have the ref range forT3.

T4 is very low on the range. Do you have any hypothyroid symptoms?

that [ 11.5 -41] doesn't look like it could be the correct range for an fT4 test. i think the 41 must be a typo Waterhouse , can you check the fT4 range you wrote ?

No carbimazole at the moment. Consultant tapered dose down, I have been euthyroid until blood test this week.

WH,

Free T4 range is 11.50-41.

11.50 - 41 is a huge range. Can you confirm this is correct.

When were vitamin D, folate, ferritin and B12 last tested

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested.

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

Low vitamin levels are extremely common, especially with autoimmune thyroid disease, Graves’ disease or Hashimoto’s

Was thyroid blood tested early morning, ideally before 9am

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

Main issues are feeling a bit edgy/anxious and intentionally trying to lose weight but losing more than expected.

Yes, bloods were done before 9am.

16 is still only halfway through the range. My T4 is halfway as well and I'm still enduring hypo symptoms....Anxiety or edginess as you mentioned, is also big hypo symptom for me. You may have brain fog as well but dont realize it because you've gotten used to it.

TSH 0.17 [0.3-5]

fT4 16.2 [11.5-22]

fT3 5.7 [ no range ]

Your fluctuating thyroid history is very interesting.

As for whether your current levels are enough to give you symptoms of either hyper or hypo thyroid is very hard to say . But if your results have changed and you feel different then yes it's possible you might start to feel hyperthyroid symptoms even though the results are still in range.

What have your 'Euthyroid' TSH /fT4 /fT3 results been like for the last (? couple of years) with no levo/ no carbimazole ?

if TSH is now significantly lower ,and fT3 is now higher than when 'euthyroid' , then the lower TSH could be an early indication that you are going overactive again,, but it's to soon to know if that is happening . you will have to keep an eye on what happens to TSH /fT3 /FT4 at next blood test. and monitor symptoms carefully .

Did you ever find out whether you have any Graves antibodies (TRab ) as these can be responsible for thyroid fluctuating between hypo / hyper / euthyriod and they could also be an explanation for your 'eye problems'..

i know you originally had a report that "thyroid antibodies ~normal", but at the time you were not sure which antibodies were tested .

"I have a multinodular goitre for the last 21 years. It’s never given me a problem (was on low dose Levo for borderline hypothyroid for 10 years) until 3 years ago, when after a hysterectomy it suddenly became overactive leaving me unwell. I was started (on propanolol )and 40mg Carbimazole, over the 3 year period levels have fluctuated

~ carbimazole history~ in Feb 2016. , was put on 40 then it was reduced to 20. Now (July 2016) it's up again, so I'm on 40 for 2 weeks then reducing to 30. Latest labs are as follows:

TSH less than 0.10 FT3 7.8 FT4 24.9

.......(Nov 2018) and I’m now on 15mg and Endo doctor is saying it’s stable and levels are coming within range."

Then euthyroid .... no carb / no levo.

How long have you been off the carbimazole for ?

Did you ever get a firm reason for you hyperthyroid phase .. ie. did they do a scan and actually find a 'hot' nodule producing too much T4/T3 ?

Have you ever read this ? ... it's an introduction to how the different kinds of TRab antibodies can cause fluctuations in thyroid conditions. I don't know if it's relevant to you or not ~ it might not be if you definitely have a 'hot' nodule.

.... but you might find it interesting anyway because it's useful to understand the reasons why some people do have fluctuating thyroid conditions ... when people don't know this , they say it's not possible to go from hypo to hyper and be euthyroid again for a while.... but even though it's unusual it does certainly happen , as you've discovered.
