Ferritin is good.
Folate is low at 4.84 (>3.89). Their range is actually 3.89-19.45 but I don't know why they don't show this any more. Folate should be at least half way through range.
Active B12 at 33.1 is dire. Active B12 below 70 suggests testing for B12 deficiency according to Viapath at St Thomas' Hospital:

If you do then list them to discuss with your GP and ask for testing for B12 deficiency and Pernicious Anaemia. Do not take any B12 supplements or folic acid/folate/B Complex supplements before further testing of B12 as this will mask signs of B12 deficiency and skew results.
Vit D is dire at 39nmol/L. This is in the "insufficient" category and your GP may or may not offer prescribed D3. If not you should supplement yourself. If your GP wont prescribe, and he probably wont prescribe enough, then come back for suggestions.
The Vit D Society and Grassroots Health recommend a level of 100-150nmol/L, with a recent blog post on Grassroots Health mentioning a study which recommends over 125nmol/L.
Thyroid Hormones:
Did you do your test as we advise:
* Blood draw no later than 9am. This is because TSH is highest early morning and lowers throughout the day. If looking for a diagnosis of hypothyroidism, an increase in dose of Levo or to avoid a reduction then we need the highest possible TSH
* Nothing to eat or drink except water before the blood draw. This is because eating can lower TSH and coffee can affect TSH.
* If taking thyroid hormone replacement, last dose of Levo should be 24 hours before blood draw, if taking NDT or T3 then last dose should be 8-12 hours before blood draw. Adjust timing the day before if necessary. This avoids measuring hormone levels at their peak after ingestion of hormone replacement. Take your thyroid meds after the blood draw. Taking your dose too close to the blood draw will give false high results, leaving any longer gap will give false low results.
* If you take Biotin or a B Complex containing Biotin (B7), leave this off for 7 days before any blood test. This is because if Biotin is used in the testing procedure it can give false results (most labs use biotin).
Your TSH is saying undermedicated but your FT4 is good, which makes me think maybe you took your Levo before the test?
Have you spoken to your GP about your frequent urination? Maybe urine infection?

High leucoytes indicate infection.
I have empathy with you. I’m new and haven’t had thyroid blood tests yet since starting the meds. I’ve also got UTI so every night broken by toilet visits. When I feel the way you describe I find a poached egg on toast is the practical thing which works quickly and gives you time to think straight.

Take these results to your GP
You need full testing for Pernicious Anaemia before starting on B12 injections
TSH is very high
Get GP to retest
Also get another test for UTI

Was this test done early morning, ideally before 9am and last dose levothyroxine 24 hours before test
Do you normally take levothyroxine waking or bedtime?
How much levothyroxine are you currently taking
Do you always get same brand levothyroxine at each prescription
You could try splitting your levothyroxine into 2 doses …..waking and bedtime…..may help improve conversion rate
Yes I followed the rules. I take actavis 75 mg. Doc lets me have 2 50s so I can half a 50 to make 25 this is to avoid mixing brands as actavisdon’t make 25 I’m still waiting to have nhs bloods for low b12 and folate and vit d but I can’t get them done before 1 June and my symptoms are worse I hve tingling in my hands and feet and I am so tired I c@n barely function. I will try splitting dose maybe 50 and 25 I have no thyroid Also my high blood pressure is not high which I find strange as it is always high I’ve had to stop medication for blood pressure. My heart rate is really spiking high and then dropping I’m at blood pressure clinic on Tuesday
