Generally, 1 grain is 60mcg and contains 38mcg of T4 and 9mcg of T3.
60 mg.

I don't think there is an easy answer to this question as it all depends on how well you are able to convert the thyroid hormone replacement.
I grain NDT generally contains around 9 mcg T3 + 38 mcg T4 ( brand dependent ) and said to be the equivalent of around 100 mcg T4 :
There again there are several variables as 100 mcg T4 will not have the same efficacy for all patients using it.
Allow the NDT levels to drop a little before adding back in the T4 and monitor on blood pressure, pulse and temperature as the T4 builds back up in your system.
Were you ever well on T4 monotherapy, and what was your dose then ?
Thank you Pennyannie. I dont think I ever tried Levothyroxine at the start of medication 6 years ago.
I was another one here going from fighting fit 2009 to virtually housebound 2010 on, with in range NHS TSH. Stumped up for private test to reveal Hashi's, so had to self medicated with NDT in 2015 before NHS agreed to a low does of Levo. more than a year after private diag. So I did not actually try Levo.
Since further finding Dio2 + Endo then agreed to trialing a little T3 to the mix. Over 6 years I have tried various combos, various different doses, timings, all adjustment carefully monitored to timings,spacing and amounts advised here.......but still no sweet spot, still unable to store energy and walk to the end of a road , and still up half the night (as you see/ read) with chronic gut pain.
Am for 6 years GF, DF, grain Free,Soy free, no fluorides,well cooked brassicas, iron, D, B's, folic all in order now, take HCl and Enzymes...........just wondered if I should try Levo....or B12 injections (in case this 'functional B12 deficiency'but ie serums,homocysteine and mma all look ok , but no explanation for flat villi ,), or HRT although long past the latter, its a possibility.
So cheesed off, I thought perhaps I should try Levo +Lio,

I guess with an endo at the helm you may never have been on enough T3 and if the TSH was deemed all important it would have been doomed to failure.
I wonder if T3 only is worth a thought ?
I don't know really and out of my depth, especially as you have these stomach issues.
Guess you know of Dr Izabella Wentz - who has written extensively on Hashimoto's.
I'm just so lucky that NDT suits me, it's not 100% but a darn site closer than T4 monotherapy.
Thank you Pennyannie, I did try the T3 regime for 6 months last year by gradually reducing out the Ndt and gradually introducing the T3, I became so weak I could barely stand up for more than 30mins daily and could only tolerate the increase up to 14mcg daily of T3, After 6 months I had to concede I need 'some' T4 too, so have gradually eased it back in as a combi of NDT and T3 ,which means I can stand up for 3 Hrs + per day but no stored energy and just pottering around..........somethings amiss....
Yes I have read all Dr Wentz books and info etc + followed through on her and about 20 other experts books on the subject......back to the drawing board for me.

Based on weight calculations, I was taking only 75mcg of Synthroid(t4) before embarking on NDT. Currently, I'm on 1 grain of Erfa plus 12.5 mcg of Synthroid but I haven't tested this dosage change yet. Ive been slowly dropping my t4 as I have been increasing my ndt and testing. For my body and how it absorbs , I feel like 30mg of ndt Erfa is equivalent to around 25 to 37.5 mcg of synthetic t4
NDT contains roughly 39mcg of T4 and 9mcg of T3 plus other things that a natural thyroid produces.
