Help advise about Thyroxine brands

I am on 75mcg Teva brand for the past few months, and I've told them it needs to be Teva as lactose free. However my new pharmacy have sent me Accord and Wockhardt (see photo). Are these lactose free?

I am strictly lactose/dairy/egg, gluten, soy and corn flour free as of a few months ago. I don't like the idea of switching between brands, although I do intend to request a different type of lactose levothyroxine as I don't feel I'm doing very well on Teva. I was previously on Mercury Pharma for many years and only switched due to the lactose.

Are these lactose free?

No. Check the patient information leaflet, it should give the ingredients.

Besides Teva, Aristo is the other lactose free brand but only comes in 100mcg tablets, although Northstar 25mcg Levo is made by Teva so will be lactose free.

My medicines document has a little table of makes and a few critical ingredients (like lactose) for UK levothyroxine and liothyronine products.

helvella - Thyroid Hormone Medicines

I have created, and try to maintain, a document containing details of all thyroid hormone medicines in the UK and, in less detail, many others around the world.

I've not seen anything reported on the forum about problems with Teva T3. The problem with Teva Levothyroxine is most certainly the mannitol they use instead of lactose so it's more a case of looking at the exipients than the brand. I use another medication made by Teva for something non-thyroid related and it's absolutely fine.

SeasideSusie thank you for this post . Recently my pharmacy wanted to switch my T3 from Mayne Pharma to the Teva and told them to switch it back to the Mayne. Would you know SeasideSusie what the fillers are in Teva please. I am very sensitive to fillers . So I was afraid to start with a new T3 that I know nothing about. But heard on the forum that the T4 is not on the preferred by many.

You won't find the information directly in my medicines document, but you should find links to everything.

I don't have the list myself as I don't use Teva so don't have a patient information leaflet but ThyroidUK's website shows this although it will be for Teva T3 dispensed here in the UK, I don't know if anything is different from the Teva T3 you get dispensed in the US:

thyroiduk.org/if-you-are-hy...

and helvella 's document is here, it is much more comprehensive and if you scroll down to page 119 Teva T3 is listed in the US section and gives a link to FDA Daily Med which gives more information so I assume you'll find the ingredients in your Teva T3 there:

I don't know where you are seeing any reference to BW.

But BW = bodyweight.

You need to make sure your surgery have this requirement on your records

I’m on just Teva. A lot of people do not get on with Teva, but I’m lucky, I do and it means that I’ve got a lacto free version of Levothyroxine. If the pharmacy jump about giving you any old brand that they’ve got in at the time this could have an effect on your health - consult with your local pharmacy and doctors surgery to find out if the brand you need has been assigned.

I’m lucky our local surgery has a meds management team who are great and I can talk to them directly and I do get sense from them more so than the GPs actually (as a whole). With a simple explanation that I was avoiding lactose and apparently it’s a good idea to be on the same brand all the time anyway, the meds management team put on my notes that Teva was the required brand. Also doubled up by asking the Pharmacy to put the same note on my records there.

I have had to check a few times and give meds back because the pharmacist has not read the information properly but I think we’ve got over that hump now. I have had to wait a day or so whilst they correct the error, but I have never given in and just taken whatever was in the bag and I would advise you to do the same.

New guidelines for GP if you find it difficult/impossible to change brands

gov.uk/drug-safety-update/l...

If a patient reports persistent symptoms when switching between different levothyroxine tablet formulations, consider consistently prescribing a specific product known to be well tolerated by the patient. If symptoms or poor control of thyroid function persist (despite adhering to a specific product), consider prescribing levothyroxine in an oral solution formulation.

academic.oup.com/jcem/artic...

Physicians should: 1) alert patients that preparations may be switched at the pharmacy; 2) encourage patients to ask to remain on the same preparation at every pharmacy refill; and 3) make sure patients understand the need to have their TSH retested and the potential for dosing readjusted every time their LT4 preparation is switched (18).
