Hi, if you are overweight, the best thing you can do for prediabetes is to lose it. With a low carb diet, you can get rid of visceral fat (fat around the organs) which is responsible for insulin resistance. I was diagnosed with diabetes (HbA1c 48 and above) some 8 or so years ago, I’ve been in remission with simple lifestyle changes (intermittent fasting, low carb diet and exercise) for most of that now. My HbA1c is now 34.

Hi,Mino40 thanks for your reply I'm 5ft 5 inch tall and my weight is 83 kg I try my best to loose it but with no luck it's more I have put weight on my belly which I never had it before I have one wholemeal toast with one egg and cup of tea in the morning then during lunch I have a banana or one sandwich then 4:30 / 5 ish in the evening I have my last meal of the day which I cook everything myself from scratch if I cut down carbs too much I get really low blood sugar symptoms and gastric pains I have tried everything to get over those symptoms but nothing works except normal meal like eating rice or chapati I been asking and complaining about these symptoms for a long time but doctors don't pay any attention to it I had no health issue prior to my Graves disease diagnosis and now one after the other

TSH, FT4 & FT3 are all in range a doctor will be satisfied you don’t require treatment.

TSH 1.56 (0.27 - 4.2)

FT4 14.8 (11.2 - 20.2) 40%

FT3 4.6 (3.2 - 6.8) 38%

They are in the lower half of range at FT4 at 40 % & FT3 38% they are well balanced.

Ferritin 62 (30 - 300) is not optimal often said should be half way through range.

HBA1C 42. (20 - 41)

Do you understand how the HbA1c measurement works? It measures the sugar attached to the haemoglobin, so it gives an average reading from past few months not a current reading.

This is range often used

* HbA1c below 41 mmol/mol (6.0%): Non-diabetic

* HbA1c between 42 and 47 mmol/mol (6.0–6.4%): Impaired glucose regulation (IGR) or Prediabetes

* HbA1c of 48 mmol/mol (6.5%) or over: Type 2 diabetes

Usually type 2 diabetes is diagnosed when 2 high HbA1c results above 47 are recorded 3 months apart. is this the first high result you have had? Retest in 3 months as you are marginally in the pre-diabetic range.

Reducing carbohydrates in diet will reduce level.

Do you feel well? Are you having any symptoms?

Not very knowledgeable about other results but they appear to be in range.

Usually high thyroid levels causes a rise blood sugar and low levels can cause them to be low.

I think your diet although healthy with fruit & wholemeal bread may not be ideal and higher in carbs than you think & you also need to make sure you get enough calories in evenly throughout the day.

A banana medium banana contains 23 carbs. I did 80 carbs a day (1200 calories) when diagnosed type 2. This is moderately low & not keto type low. So it’s over a quarter of your allowance.

chapati, bread & rice are also very high.

What are your low blood sugar symptoms. Are you sure it is low blood sugar? Do you have a monitor or are able to do finger prick test? Could the symptoms be from another cause?

I think you should do a food diary and pin point which food cause the most issues and take it to your doctor.

Could you have issues with gluten?

Could you have low stomach acid & not high stomach acid?

First thing in the morning, before eating or drinking anything:

1. Mix ¼ tsp baking soda in 4 to 6 ounces of cold water.

2. Drink the baking soda solution.

3. Time how long it takes for a burp to occur. Time it for up to 5 minutes:

4. If you have not burped within five minutes, it may be a sign of insufficient stomach acid. Early and repeated burping may be due to too much stomach acid (do not to confuse this with small burps from swallowing air when drinking the solution). Any burping after 3 minutes is an indication of low stomach acid levels.

This is what I would do

Make sure I had portion of Milk or yogurt each day,

Go for berries which are better than other whole fruit, NO juice.

2x a week small amount of oats but very little bread, rice, or cereal.

3-4 a week small portion of amount of Legumes Beans and other plant-based proteins

Lots of salad & Vegetables, but no Potatoes or corn

1 small piece of chocolate or treat most days but no cakes, biscuits & desserts

Small portion or nuts every day, eg 3 Brazil’s or 5 almonds.

meat, fish and an egg most days.

I used an app eg MyFitnessPal or fit bit to track my carbs, doing that really gets you to understand where you are consuming carb & getting carbs you weren’t aware of & while you learn you gradually adjust your diet in the right way.

Most apps will calculate a calorie count based on current weight / height and weight loss aim, but the minimum calories is 1000.

Hi,Purple Nails I don't have a monitor to check my sugar levels symptoms like in the morning when I open up my bowels I feel nauseous afterwards low in energy if I don't eat within an hour I feel woozy and lousy cannot think straight ..Acid level when I drink one full glass of water first thing in the morning without even using baking soda trick I burp within the seconds I tried Apple cider vinegar , digestive enzymes so many things nothing works apple cider vinegar was making my gastritis worse so I stopped it.i believe in my case it's not low stomach acid it's high.so many foods I have avoided eating now .Gluten when I had endoscopy in 2018 my gastro took my stomach sample and I was negative for coeliac my whole digestive issue started after I was diagnosed with Graves disease and I was only on carbimazole for 8/10 weeks then stopped completely by the doctors saying I'm becoming hypo now even I was complaining about the symptoms since 2016 when I was diagnosed with Graves disease I never felt myself again

That’s a short time to go in remission for Graves.

I read on previous post you had a scan which shows cysts & nodules. Have you had a uptake scan to access functional areas of thyroid. Any further tests on thyroid?

I see you have positive TRab confirming Graves and borderline high Thyroid Peroxidase (TPO) & Thyroglobulin antibodies (TgAb) which are most associated with Hashismotos. You may have both autoimmune conditions & hyper & non functioning nodules causing lots of confusing fluctuations.

TSH will not be reliable how frequently is FT4 & FT3 tested? I suspect Like many of us you don’t have records of when your thyroid levels were normal for you. Are you able to use monitor my health which test FT4 & FT3 every 2 to 3 months to gain a records of what your levels are doing, if they are stable or not.

I hadn’t picked up you have eye issues. Any of the following?

● Change in the appearance of the eyes (eg staring or bulging eyes)

● A feeling of grittiness in the eyes or excessive dryness in the eyes

● Watery eyes

● Intolerance of bright lights

● Swelling or feeling of fullness in upper or lower eyelids

● New bags under the eyes

● Redness of the lids and eyes

● Blurred or double vision

● Pain in or behind the eye, especially when looking up, down or sideways

● Difficulty moving the eyes

If you do ask for a referral to thyroid eye specialist in ophthalmology in case you have TED thyroid eye disease which can developed preceding, during & after treatment of Graves.

Hi.Purple Nails thank you so much once again no I have no idea what my thyroid results were before diagnosis ...I never had uptake scan of my thyroid my GP sent me this year for scan in June and radialogist told me that I have benign noudles and cysts which is not concerned about and she said thyroid is showing thyroditis which I have no idea .I already have ordered monitor my health kit.myt4 and T3 doesn't move at all but my TSH goes up and dow little bit last tshresult was few months back 1.93 with the same lab range before I think it was 2.15 ..I had gritty eyes when I was diagnosed iwas under the care of opthomologist but they discharged mecouple of years back saying I don't have Ted ..I don't have bulging eyes or bags under my eyes ..but yes I used to get pain behind my right eye and I don't like bright lights anymore.

Thyroiditis is the term for autoimmune Hashimoto’s, a visual scan image is conclusive diagnostically. If the FT4 & FT3 are stable and TSH respond well to levels I’m afraid the doctors will not treat until the results show as abnormal. I would still test fairly frequently to monitor for any trend downward or upward.

If you still have eye issues persist with doctor, & reiterate you previously had positive TRab the bio marker for TED. You may find high street optician are more helpful and can write a letter to pass to doctor. See 2nd half of my profile I have eye issues & am still battling with doctors.

Hi,thanks I will attach the picture of my ultrasound .on my FB thyroid forum page when I asked the same question about thyroditis someone said it's the term they use for the inflammation of thyroid I get confused because every time they did my ultrasound of thyroid they reported thyroditis

Scan

PurpleNailsAdministrator in reply to Samy3

Ultra sound scans are useful for detecting nodules and appearance but doesn’t determine functioning parts of thyroid . They are relying on thyroid function test for that.

The report does confirm the appearance of thyroiditis. Intermittent swelling & inflammation would occur as the level of immune system attack would fluctuate.
