Newbie…. Looking for advice, help on test results please🙏🏻

Hi I’m new here and was hoping someone could help me with some results I’ve had back from Monitor my Health? After years and years of feeling terrible and going back and forth to my GP I decided to change doctors when he diagnosed me in 2020 with hashimotos. Since then I have been on 100mcg of levothyroxine from the brand Almus. I take last thing at night. I know that the GP won’t test t3 so I did a test with monitor my health and these are the results. Can anyone tell me if it’s ok? I still feel rubbish really but GP says my results are normal. Any help at all I would be so appreciative.

Sorry here are results

0.1 mu/L

TSH levels low

(normal range 0.27 - 4.2 mU/L)

18.9 pmol/L

FT4 levels normal

(normal range 12 - 22 pmol/L)

4.8 pmol/L

FT3 levels normal

(normal range 3.1 - 6.8 pmol/L)

Welcome to the forum

So presumably you took levothyroxine as per normal at bedtime, then tested the following morning?

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test so that you leave 24 hours between last dose levothyroxine and test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

When were vitamin D, folate, ferritin and B12 last tested

What vitamin supplements are you currently taking

As you have Hashimoto’s have you had coeliac blood test done

Are you on strictly gluten free diet or dairy free diet

Hi thanks for responding. I missed my levo the night before blood test and just took it as soon as I came back out the doctors. I’m due to have my bloods done again tomorrow and it’s checking vitamins etc. I did pay for a celiac test but came back negative. I did ask my GP about getting tested but he said no need😳 My Vit D is always up and down but I’ll post as soon as I get results next week. I’m actually trying a gluten free diet at the moment, well it’s only been two weeks, but not dairy. Does my T3 look ok to you? Is there a reason why GPs don’t test T3? And is it normal to be so fuzzy in your head all the time. My GP wanted to put me on antidepressants but I sooo not depressed. I’m just fed up with feeling so rubbish and being told everything is normal. Sooo frustrating

So your Ft4 is falsely low as you left 36 hours between last dose levothyroxine and test

So this suggests poor conversion of Ft4 to ft3

Ft4 would be higher at 24 hours…..likely at least around 21

Ft3 is only 46%through range

First steps are

Trialing absolutely strictly gluten free. Have you noticed any improvements yet? (It can take many months for brain fog to lift)

Getting vitamins optimal

Vitamin D at least around 80nmol and around 100nmol maybe better

Serum B12 at least over 500

Folate and ferritin at least half way through range

What vitamin supplements are you currently taking

As to why NHS doesn’t routinely test Ft3….they say it’s irrelevant

Cynics might say …..it’s to avoid having to prescribe T3 to patients who have low Ft3

Vast majority of endocrinologists are diabetes specialists and won’t prescribe T3

Roughly where in U.K. are you

Email Thyroid UK for list of recommend thyroid specialist endocrinologists who will prescribe T3 if necessary

I live in Northamptonshire, and I don’t take any supplements at all. I haven’t noticed any difference yet so hopefully in a few more weeks I will. So have a cocked up for my test tomorrow then? I’m due to have bloods done at 9:30. My last B12 was:

Serum vitamin B12 level 968 ng/L [197.0 - 771.0]

So my GP said I dangerously high, cause at the time I was taking Holland an Barrett timed release 1000ug. That’s why he told me to stop taking any supplements at all, so I did. And my folate and ferritin were

Serum folate level 3.4 ug/L [3.0 - 20.0]

Serum ferritin level 40 ug/L [13.0 - 150.0]

And my doctor said I don’t need to see an endocrinologist as I’m in normal ranges. But I feel awful.

GP’s learn nothing about the importance of good Vitamin Levels on levothyroxine

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial. This can help keep all B vitamins in balance and maintain B12 levels too

Many Hashimoto’s patients have MTHFR gene variation and can have trouble processing folic acid.

B vitamins best taken after breakfast

Thorne Basic B or Jarrow B Right are recommended options that contains folate, but both are large capsules. (You can tip powder out if can’t swallow capsule)

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

Serum TSH level < 0.05 mu/L [0.27 - 4.2]

Serum free T4 level 20.2 pmol/L [12.0 - 22.0]

Serum free triiodothyronine level 4.4 pmol/L [3.1 - 6.8]

Blood haematinic levels

Serum vitamin B12 level 286 ng/L [197.0 - 771.0]

Serum folate level 2.0 ug/L [3.0 - 20.0]

Serum ferritin level 28 ug/L [13.0 - 150.0]

I left more than 24h between my levo so I know that’s not going to be accurate but any advice I’d be so grateful 🙏🏻

Only Look at increasing iron rich foods in diet

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.
