Miscarriage and Hypo - what next to improve my chances of a successful birth?

hi everyone - you may remember my previous post about pregnancy and hypothyroidism.

I went for my 12 week scan yesterday and they’ve informed me the baby passed - 8.5 weeks measured and no heart beat. Just have to wait for baby to pass naturally.

I am 35 so keen to try to conceive as soon as possible and wondering if you guys have any advice on what I should now test in bloods.

I am worried I have caused this as I was going gluten free for months before becoming pregnant but at week 8 I had to eat gluten as I couldn’t tolerate gluten free bread with the nausea from pregnancy…

Sad times and would welcome any advice. Thank you x

Your TSH was likely too high, or vitamins too low ….

Give your body time to recover.

Working on improving low vitamin levels

Vitamin D and B12 were low in last post

Ferritin at 35 was especially low

GP should be doing full iron panel test for anaemia

It’s important that vitamins are optimal BEFORE conception, especially folate and B12

A baby’s Neural tube is formed extremely early in pregnancy

B vitamins are Goldilocks vitamins, you need the right levels….not too little or too much

Suggest you retest thyroid and vitamin levels in 6-8 weeks

Always test as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24 hours before test

How much levothyroxine are you currently taking

Do you always get same brand levothyroxine at each prescription

Hi there - I am now on 200 mg of Levo after going against my docs advice. I always get the same brand. Current TSH is 1.3 a couple weeks ago. Folate is now over and b12. How do I get more ferritin?

That’s likely too large an increase

How much levothyroxine were you taking BEFORE pregnancy

Ferritin

Look at increasing iron rich foods in your diet

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

I was taking 150 which resulted in 2.4 TSH - I went from 175 to 200 quite quickly. Do you think that may have caused my miscarriage?

Probably not

Miscarriage is very common, sadly

So if you were on 150mcg before pregnancy, you might only need to be on 175mcg

Bloods should be retested 6-8 weeks after being on constant unchanging dose and brand levothyroxine

Thanks SlowDragon - they have said I am not to have another test until three months but I am going to get a private one done in a few weeks time to test all the above vitamins and my full thyroid. I am curious how my antibodies are doing after I stopped running during pregnancy

My levo is ALMUS - have you heard of it?

Most easily available (and often most easily tolerated) are Mercury Pharma or Accord(Almus )

Mercury Pharma make 25mcg, 50mcg and 100mcg tablets

Accord is also boxed as Almus via Boots, and Northstar 50mcg and 100mcg via Lloyds ....but Accord doesn’t make 25mcg tablets

beware 25mcg Northstar is Teva

Once you find a brand that suits you, best to make sure to only get that one at each prescription.

Watch out for brand change when dose is increased or at repeat prescription.

If a patient reports persistent symptoms when switching between different levothyroxine tablet formulations, consider consistently prescribing a specific product known to be well tolerated by the patient.

Physicians should: 1) alert patients that preparations may be switched at the pharmacy; 2) encourage patients to ask to remain on the same preparation at every pharmacy refill; and 3) make sure patients understand the need to have their TSH retested and the potential for dosing readjusted every time their LT4 preparation is switched (18).

Levothyroxine is an extremely fussy hormone and should always be taken on an empty stomach and then nothing apart from water for at least an hour after

Many people take Levothyroxine soon after waking, but it may be more convenient and perhaps more effective taken at bedtime

No other medication or supplements at same as Levothyroxine, leave at least 2 hour gap.

Some like iron, calcium, magnesium, HRT, omeprazole or vitamin D should be four hours away

(Time gap doesn't apply to Vitamin D mouth spray)

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

REMEMBER.....very important....stop taking any supplements that contain biotin a week before ALL BLOOD TESTS as biotin can falsely affect test results - eg vitamin B complex

So sorry to hear your sad news. But, you mustn't blame yourself. I very much doubt eating gluten had anything to do with it.

That is a relief I am just worrried as they say I now have fibroids and I wondered if these were caused by gluten and somehow compromised my baby. I guess it’s only natural to blame yourself but as days go on I am gaining perspective. Thanks so much for your reply
