Ok. I have a 15 yo daughter. She has been overweight her whole life despite what I fed her. She is very tall, and a solid girl. She started puberty when she was 7. She’s suffered with constipation her whole life. Diagnosed autistic at 7. Has severe anxiety and very poor self esteem. Has been self harming for 4/5 years. Hates her body, hates the fact she can’t slim down even when she eats so much less than her peers and walks 5 miles in a school day. Has no energy, very lethargic, very slow! Brain fog, can’t concentrate.
Finally managed to get her seen by NHS endo. It’s been a frustrating experience to say the least. Blood tests in September showed she has hyperinsulimia. (I don’t have the results) Her TFT was out of range:
T4 10.2 (ref range 12-22)
TSH 6.2 (ref range 0.27-4.2)
I have a very quick follow up conversation on the 11th March, he wants her to have further thyroid tests and start metformin. Still waiting for the letter as he’s not actually signed it off yet so she can’t start metformin and we can’t get any further investigations done. Nearly a month ago.
My GP practice did a TFT, came back ‘normal’
T4 13.5
TSH 3.59
Obviously she needs antibodies tested. And metformin. And I am certain she is hypothyroid, I am too. I am so angry that her results weren’t acted on back in September, now we’ve reached crisis point and she’s been making herself sick and even overdosing on paracetamol- not as a suicide attempt but because she wants to go in to a coma as she feels she has no control over her physical or mental health. I only found this out last week. Obviously medication has been locked away. And we are desperately trying to get her the help she needs (been trying for the last 5 years 😩)
I think I need to go private as this is taking too long and she needs help. Can anyone recommend private paed endos in the south east please. Someone who will actually listen to her.

So sorry your daughter is going through this, it must be very worrying for you.
Clearly she is hypothyroid as her September blood tests show and even though her GP results are within range her TSH is still far too high and in some countries she would get a diagnosis as it's over 3.

For full Thyroid evaluation she needs TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also EXTREMELY important to test vitamin D, folate, ferritin and B12
Low vitamin levels are extremely common, especially as you have autoimmune thyroid disease (Hashimoto's) it’s likely she might too
Ask GP to test vitamin levels
Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .
This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)
Private tests are available as NHS currently rarely tests Ft3 or thyroid antibodies or all relevant vitamins
If TPO or TG thyroid antibodies are high this is usually due to Hashimoto’s (commonly known in UK as autoimmune thyroid disease).
About 90% of all primary hypothyroidism in Uk is due to Hashimoto’s. Low vitamin levels are particularly common with Hashimoto’s. Gluten intolerance is often a hidden issue to.
