Need private endocrinologist

Im at a wits end with my thyroid issues & my gp’s failing to understand how i am feeling. My TSH has always fluctuated the highest being at 7.1 last year in june 2020. Iv had repeat bloods, and iv been classed as subclinical hypothyroid & been told no meds are required.

I did a private test with medichecks in oct 2020 and the issue of antibodies was bought to light, yet gp said nothing to worry about. See attached photo. Since oct 2020 -feb 2021 my tsh has gone 4-5.43.

Iv had a constant sore throat since june last year, iv had numerous docters & ent appointments. Also had a gastro endoscopy in nov 2020 as my neck & throat was in pain all the time. All was clear.

I am now loosing so much hair to the point that long shreds are coming out in clumps, & im starting to see patches of no hair on my scalp. I feel really low, have lost so much weight (not typically hypothyroid) , constantly cold. The list goes on.

Out of desperation I went today to get a private ultrasound of my thyroid as my neck and throat is constantly in pain. This has never been offered by my gp’s. I was told by the radiologist that my thyroid is highly inflamed & will receive a written report by tomorrow am.

Im just so tired of feeling like this and was wondering if a private endocrinologist or even gynaecologist appointment would be better in understanding what is happening. I live in london, so any suggestions would be much appreciated.

Many thanks.

How long ago was this test done

Was it done as early as possible in morning before eating or drinking anything other than water

If this was 2nd abnormal test ....with TSH above range and hypothyroid symptoms, GP could have started trial of levothyroxine. Especially as you have high thyroid antibodies

Important to improve low vitamin levels by supplementing

Vitamin D insufficient and GP could/should prescribe vitamin D at 1600iu daily for minimum of 6 months

B12 too low, but not low enough for GP to treat

No folate result

Ferritin also too low

Low vitamin levels tend to lower TSH

Many thanks for your reply. Yes all bloods were early morning before 9am without food. Since the medichecks bloods above i have been supplementing vit d, b12, eating iron rich foods as well as cutting out gluten. My iron levels are still low despite eating iron rich foods, so am now supplementing. Iv had repeat tests with gp since October 2020. But my gp believes that i don't need meds as my TSH is always fluctuating in between the ‘normal’ boundaries. So i am now looking to go privately

More Blood test below:

Dec 2020

TSH : 4.27 (0.27-4.20)

Serum free T4 : 16.4 (12.0- 22.0)

Feb 2021

TSH: 5.43 (0.27-4.20)

Free thyroxine: 17.7 (12.0 - 22.0)

Low vitamin D

GP should prescribe 1600iu everyday for 6 months

NHS Guidelines on dose vitamin D required

ouh.nhs.uk/osteoporosis/use...

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

leedsformulary.nhs.uk/docs/...

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

mm.wirral.nhs.uk/document_u...

But with Hashimoto’s, improving to around 80nmol or 100nmol by self supplementing may be better

ncbi.nlm.nih.gov/pubmed/218...

vitamindsociety.org/pdf/Vit...

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Test twice yearly via NHS

As a Hashimoto’s sufferer, I’ve been using jarrow vitamin d for years, personal I would not be able to use the vitamin d linked above, palm oil causes headaches for me.

Low B12

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial. This can help keep all B vitamins in balance and will help improve B12 levels too

Difference between folate and folic acid

chriskresser.com/folate-vs-...

Many Hashimoto’s patients have MTHFR gene variation and can have trouble processing folic acid.

thyroidpharmacist.com/artic...

B vitamins best taken after breakfast

Igennus Super B is good quality and cheap vitamin B complex. Contains folate. Full dose is two tablets per day. Many/most people may only need one tablet per day. Certainly only start on one per day (or even half tablet per day for first couple of weeks)

Or Thorne Basic B is another option that contain folate, but is large capsule

If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

Low B12 symptoms

b12deficiency.info/signs-an...

With B12 result below 500, recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months, then once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.

Low ferritin

Look at increasing iron rich foods in diet

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Thyroid disease is as much about optimising vitamins as thyroid hormones

You need to see a private endocrinologist ASAP . Your thyroid needs treating with meds . No doubt, your Gp is not a specialist and I can’t believe you aren’t on thyroid meds . My daughter was misdiagnosed for 10 years and she had thinning hair, dry skin , freezing all the time , tired , couldn’t lose weight no matter what she did and now she is completely on track : levels normal hair full weight back to responding to exercise and healthy diet instead of gaining when she was doing everything right. She is on thyroid meds. Your thyroid panel needs an expert hormone specialist . Vitamin D3 needs supplementing as well as mine was low and they had me supplement abc mine wasn’t ass low as yours. Order the magnesium miracle by Dr Carolyn Dean and start taking magnesium glycinate daily . Her book is full of information you will find really helpful as well as wow! Also, go on Instagram or Facebook and search through Dr Mark Hymens posts about thyroid very very informative and really insightful. Overall you need second opinions and your bloodwork is worthy and absolutely necessary to seek that out! I’m a mum who has fought through to get a dr who changed her life and it was our new GP in the end who listened and treated over finally. Reading thyroid panels etc is tricky and yours are in need of a so change drs and move on as this needs a dr who knows and understands thyroid levels and symptoms. It’s not right you are losing weight and so cold. Optimal Vitamin D3 the book is another and thyroid issues and low vitamin D3 are related and your magnesium. But you need thyroid meds as well . Hope this helps hang in there but don’t settle this is your health and hair that’s falling out is a sign of you’re not crazy but rather you’re in need of a specialist . I feel so bad for you . Take care of you 💛
