Hello,

I spoke with my GP about managing menopause symptoms, night sweats, insomnia with my hypothyroidism. I had my bloods and vitamin D taken after fasting.

My serum TSH level is 0.13 (0.27 - 4.20) it was 0.72 in August

Vitamin D was 54 (25 -50)

The GP reported that the serum TSH level is stable from the previous result by It has dropped by 0.6. He also said that I had Graves not Hashimotos as I had believed.

Do I challenge the GP with regards to the TSH being stable?

No, you challange him about the Graves. What is his reason for saying you have Graves? Did he test the TSI or the TRAB antibodies?

If you have Graves' disease your TSH will be suppressed mine was 0.002. What is the lowest TSH you have had?

Suggest you don’t bother to see that GP again

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also EXTREMELY important to test vitamin D, folate, ferritin and B12

Low vitamin levels are extremely common, especially if you have autoimmune thyroid disease (Hashimoto's) diagnosed by raised Thyroid antibodies

Ask GP to test vitamin levels

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

If/when also on T3, make sure to take last third or quarter of daily dose 8-12 hours prior to test, even if this means adjusting time or splitting of dose day before test

Is this how you do your tests?

Private tests are available as NHS currently rarely tests Ft3 or thyroid antibodies or all relevant vitamins

Just testing TSH is completely inadequate

Vitamin D is too low

GP will often only prescribe to bring levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

Once you Improve level, very likely you will need on going maintenance dose to keep it there.
