Hope you’re all keeping safe and well. I’ve recently carried out a Medichecks thyroid panel after starting Erfa thyroid along with levo. I’ll attach a pic of my last results in August for comparison. I’m now taking 50mcg of levo and 30mg of Erfa in the morning and another 30mg of Erfa in the afternoon:

T4: 17.6 (12-22)
T3: 5.19 (3.1-6.8)
My private endo wants me to continue as I am now and retest early January. I do feel much better, fatigue has definitely lifted as well as the brain fog.
I was just wondering should I be looking to increase my T4 as it’s not in the top quarter any longer? My endo knows I’m TTC and has patients to take NTD whilst pregnant/TTC even though guidelines don’t favour it.

Well yes, your T3 has risen since your previous post so this is a positive step and in the right direction but if you still have symptoms there is plenty of room in your T3 level to increase the NDT.:
Do you intend to come off the synthetic T4 and switch totally to NDT ?
When on NDT only it's normal to see a low suppressed TSH, with low in range T4 and a much higher in range T3 so I think your T4 conversion into T3 will improve further with optimal vitamins and minerals.
I can't see too well where these levels sit, so just to say I aim for a ferritin at around 100 : vitamin D 100 + : folate at 20 and active B12 at around 70 + : serum B12 at around 500 + :
Please remember that NDT was successfully used for over 100 years to treat the symptoms of hypothyroidism and you simply dose to the relief of symptoms.
The blood test and ranges were introduced to be used alongside the new thyroid hormone replacement Levothyroxine, launched by Big Pharma to supersede Natural Desiccated Thyroid.
Thank you very much for your reply. Yes my endo has said to think about if I want to come off levo all together and transition to only NDT. I’m really unsure what to do to be honest. I’m working on my vits. I think I’ll do a vitamin and thyroid panel next time I order from Medichecks to see how they’re getting on. I can’t believe how far I’ve come to be honest. This time last year with a TSH of 28 I felt i was never going to get anywhere near where I am now, and that’s due to this wonderful forum with so many knowledgeable people. I will be forever grateful!

Personally I'd come off the T4 and switch to NDT only - and as I see it if conversion is optimal there should be little or no surplus T4 hanging around anyway.
Obviously NDT is at a fixed ratio of T3/T4 at around 1/4 T3/T4 and this seems to suit the majority of people, but I read of some people adding in a little T3 or T4 when they are very close to that elusive " sweet spot " we are all meant to find.
I'm guessing this endo is private and with the NDT costs may need to be considered :
I'm self medicating with NDT and currently there is no Far East production to speak of and supplies from the USA are more expensive.
Hopefully once you feel confident and settle down on a dose you may be able to save a few pennies also when Far East production becomes available again.
Yes he’s private and issues me a private prescription which is pricey but I would definitely find a way if it gives my life back. He was a little concerned that my TSH is suppressed but I don’t feel worried about it because I feel so much better in myself
Good to read -I have to say though that I didn't pick up on the fact that you are looking TTC and my answer takes no account of this situation, so as SD has added in, I'd take her advice over mine .

It is very subtle and I've continued to improve several months on, though I've been on the same dose for nearly 2 years now but I still need to supplement the vitamins and minerals for optimal conversion.
Yes you’re right and he has several patients which are either TTC or pregnant who are either on a combination or levo and NDT or NTD alone. I hope that when I get pregnant I won’t get any grief for taking NDT because it’s really been a game changer for me
