Obviously folate is deficient and needs supplements to improve
GP should prescribe folic acid, but you may be better supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.
This can help keep all B vitamins in balance and will help maintain B12 levels too

Many Hashimoto’s patients have MTHFR gene variation and can have trouble processing folic acid.

B vitamins best taken after breakfast
Igennus Super B is good quality and cheap vitamin B complex. Contains folate. Full dose is two tablets per day. Many/most people may only need one tablet per day. Certainly only start on one per day (or even half tablet per day for first couple of weeks)
Or Thorne Basic B or jarrow B-right are other options that contain folate, but both are large capsules
If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before any blood tests, as biotin can falsely affect test results

Magnesium - are you supplementing?
I am not supplementing with magnesium.
I take vitD chewing tablets and Clopidogrel.
Which vitamin D...perhaps it has magnesium in it. Magnesium is cofactor for vitamin D
Adcal-D3 chewable tablets calcium carbonate(1500mg) and vitamin D3(400 I.U)
Contents: calcium carbonate 600mg, vitaminD3 10 micro gram cholecalciferol, Sodium saccharin,
Magnesium sterate.
So yes ...magnesium
Surprising that your vitamin D is that good on such tiny dose vitamin D
Even though I wasn’t well for a while, it all got too much in 2011, the GPS couldn’t find the problem. I went private to an Endo who found my vit D was single figure and parathyroid was 120 plus. Thyroid antibodies 1300plus, and was told i will become hypo in the future.
I was given a high vitamin dose for for 8 weeks which stabilised the vitD and parathyroid. I then started to have physical symptoms beginning with lychen planus on both legs and the upper torso. Joint pains, muscle twitches wrist pains etc.etc. We are now where we are.

High TPO antibodies confirms autoimmune thyroid disease
Have you had coeliac blood test yet?
Going absolutely strictly gluten free often reduces symptoms, and frequently slowly lowers TPO antibodies.....but rule out coeliac FIRST by testing
You will struggle to get any medic to agree to prescribe Levothyroxine
TSH is above average, but not above top of range
Ft4 is low and total T4 very low
Ft4 is 45% through range
Ft3 only 32% through range
Helpful calculator for working out percentage through range
You might push for 3-6 month trial of levothyroxine....but many medics wouldn’t agree
Insist on ultrasound scan of thyroid....if this shows lots of damage that would strengthen your case for needing levothyroxine
Suggest you retest thyroid and vitamins in 3-6 months after working on vitamins and trialing strictly gluten free diet
The BH Doctor’s Comments:
The thyroid function is currently normal. The positive thyroid antibody result, however, increases the possibility of your having or ultimately developing autoimmune thyroid disease, such as Hashimoto's thyroiditis or Grave's disease.
The very small rise in magnesium is highly unlikely to be of significance. A repeat test in a few weeks’ time would be reasonable if you are concerned to ensure the level is not rising. A sustained higher level of magnesium may be a sign of Addison's disease, hyperparathyroidism, hypothyroidism, high intake of antacids or laxatives, or kidney disease.
The folate level is low. Folate (folic acid) is one of the B group of vitamins found in green vegetables in particular. The body's reserves of folate, unlike vitamin B12, are low and only sufficient for about four months. Causes of deficiency include reduced intake from the diet or from poor absorption through the gut and side effects of some medication (eg methotrexate). Symptoms include fatigue, mild sensation changes and depression. Prolonged lack of folate results in megaloblastic anaemia (in which the red blood cells are characteristically large). Supplementation would be advisable - I advise you to discuss this result with your usual doctor.
Magnesium high because you are supplementing
High Thyroid antibodies confirms autoimmune thyroid disease
Yes thyroid levels technically “normal “
Improving low folate essential
Strictly gluten free diet may be astounding improvement....or can be slow reduction in symptoms
