How to diagnose Hashi

Please can you refresh, which antibodies are over range to denote Hashimotto ? I thought it was Thyrog...... ones not the others? (perox.....).

.....please refresh, when you have a Hashi swing what is taking place and why?

The first group, the TPO Ab, are found raised in Hashimoto's disease - otherwise known as autoimmune thyroiditis. Here the cells of the thyroid gland are attacked and slowly destroyed. Patients with these antibodies present either have Hashimoto's, or are going to have it with subsequent reduction of thyroid function. (Elevated levels are found in virtually all cases of Hashimoto's disease and they will also be raised in 65% of patients with Graves' disease).

The next group is the TG Ab. These levels rise as well as the TPO Ab levels in autoimmune thyroiditis, but to a lesser degree.

when you have a Hashi swing what is taking place and why?

With Hashi's the immune system attacks the thyroid and gradually destroys it. When the attack happens, the dying cells release a lot of thyroid hormone into the blood and this can cause TSH to become suppressed and Free T4 and Free T3 to be very high or over range. These are called 'Hashi's flares' or 'swings'. You may get symptoms of being overmedicated (hyper type symptoms) to go along with these results that look as though you are overmedicated. The hyper swings are temporary, and eventually things go back to normal. Test results settle back down and hypo symptoms may return. Thyroid meds can be adjusted slightly at these times if necessary, but will need readjusting when hypo symptoms return.

Yes the thyroglobulin Ants were 135 when the range :15

But doc says these weren't recognised by NHS to be hashi

Medichecks range for Thyroglobulin antibodies goes up to 115 (that may just be a typo where you've put :15).

Raised Thyroglobulin antibodies alone aren't confirmation of Hashi's, it can be indicative of other things. It's raised TPO antibodies which the NHS recognises wheer Hashi's is concerned.

What does TPO stand for pls?

I'm incorrect then but what do mine over range mean? ..... Il get it posted on tmrrow with a bit of help from a friend but it was over for sure

Thyroid Peroxidase. An enzyme which usually exists only inside the thyroid and does some of the work of creating thyroid hormone. Most often we are talking about antibodies to Thyroid Peroxidase which are an indicator of autoimmune thyroid disease (Hashimoto's).

Ah thanks helvella, so it's the peroxidase antibodies, the gp was correct in her assumption, and to think I believed I'd moved into hashi all this time!! ......I'm visualising a doorstep of wholemeal toast as I write!! 😬 A member unwittingly led me this way, misunderstanding of course. And maybe gf is still beneficial

....on three tastings Iv had a little over the range, sometimes in range

Iv pondered how much this affects the gland?

I'm gf and do all the rest to heal/ tackle

So doc is incorrect re this, I just wanted my record to be accurate but seemingly it doesn't even say hypo even though I was diagnosed ( after yrs of it) in 2016

Feel free to quote, how it will go down is another thing, doctors don't attach much importance to antibodies.

I'm not Hashi's so have never experienced a Hashi's swing, but I imagine the effect varies.
