Hi, I don’t know where else to turn, looking for some advise/help. I feel like I’m talking to a brick wall with GP’s.

I have been treated for hypothyroidism for the past 17yrs. I have been taking 225mcg of levothyroxine for the last 3 years.

I have been feel ‘off’/symptomatic for the last 3 months. Symptoms getting progressively worse.

Went to GP and had bloods done.

FT4 8.2 and TSH 95.10!!!!

I feel SHOCKING. I’ve never felt so bad.

GP said he spoke to endocrinologist and a referral is not necessary. Instead I’m to increase levothyroxine to 250mcg.

I questioned my TSH (as it is so very high)

My GP told me to take my medicine ‘properly’, I explained that I do.

I explained my worsening symptoms and new symptoms.

The GP asked if I was depressed and did I want antidepressants. Like this is all in my head??

I’m not imagining how bad I feel or look.

I don’t know what to do. Any advice would be gratefully received. Thank you in advance.

Have you ever had thyroid antibodies tested - Thyroid Peroxidase (TPO) and Thyroglobulin (TG)? If not then I was would. If your TSH and FT4 were in range before then for such a high TSH and FT4 probably now below range (do you have reference ranges for the results?) then it suggests autoimmune thyroid disease aka Hashimoto's.

I don't think your GP knows much about this from what you have written and it doesn't sound like the endo does either. Most doctors dismiss antibodies as being unimportant anyway.

Your GP should have referred you

Have you recently had change in brand of Levothyroxine?

Many, many patients react extremely badly to Teva brand

Do you always take Levothyroxine on empty stomach and then nothing apart from water for at least an hour after. Essential to never ever take any other medication at same time

Do you have Hashimoto's also called autoimmune thyroid disease diagnosed by high thyroid antibodies?

Low vitamins are extremely common especially with Hashimoto's

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also extremely important to test vitamin D, folate, ferritin and B12

Low vitamin levels are extremely common, especially if Thyroid antibodies are raised

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and fasting. Last Levothyroxine dose should be 24 hours prior to test, (taking delayed dose immediately after blood draw). This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)

Is this how you do your tests?

Private tests are available. Thousands on here forced to do this as NHS often refuses to test FT3 or antibodies or all vitamins

Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have special offers, Medichecks usually have offers on Thursdays, Blue Horizon its more random

If antibodies are high this is Hashimoto's, (also known by medics here in UK more commonly as autoimmune thyroid disease).

About 90% of all hypothyroidism in Uk is due to Hashimoto's.

Low vitamins are especially common with Hashimoto's. Food intolerances are very common too, especially gluten. So it's important to get TPO and TG thyroid antibodies tested at least once .

Wow! Thank you very much!

I take vit D but that’s it.

GP won’t do FT3 or antibodies.

I think I’ll do the Medicheck bloods.

Really considering private endocrinologist too.

Hi, just because an endo is maybe private, it doesn't mean that he will be any better. You could be wasting your money. Better to get one off the list as recommended and go from there. Jo xx

Goodness, it seems as if your 225 of Levo is doing NOTHING at all so what would another 25 do for you? You may as well be on nothing. He should figure out what is blocking it. Your reverse T3 is probably very high. I hope you do look further with testing. Estrogen dominance and insulin resistance are culprits, high antibodies due to gluten, lack of vitamin support ALL the B vitamins, methylated types of B12 and folate are crucial. I'd start a digestive enzyme or HCL Betaine with meals because you probably have low acid. Minerals and amino acids need enough acid to demineralize.

You also need blood test rerun to see if it was a fault in test machine. Just to double check it wasn't an erroneous test result

Are you taking any supplements that contain biotin?

If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before any blood tests, as biotin can falsely affect test results

I’m only taking vit D and I take that separately. The test was sent to another lab because the level was so high. They got the same result.

I’m having bloods today to test for celiac.

You all know so much about it. I feel embarrassed at my ignorance of my own condition. I also feel angry that at no point over the last 17 years, not one GP has empowered me with any of this knowledge.

I was extremely ill for 24 years until I got full private testing as recommended on here

Turned out to be gluten intolerant, not coeliac

Also DNA testing recommended eventually. Many of us have genetic variation that means you need small dose of T3 prescribed
