Advice please. What do you think of my recent results. Serum vitamin B12 value 337.7pg/ml normal range 197.0 -771.0. Serum folate. Value. 4.49. ng/ml. Normal range. 3.90 - 26.80. Serum ferritin. Value. 17.2 ng/ml Normal range. 14.0 - 186.0. Free. T4 level value 26.7 pmol/L normal range 12.0 - 22.0 Serum TSH level Value 0.66 mU/L normal range. 0.27 - 4.20. I have been feeling very low fatigued etc following a virus I have asked doctor to test me for vitamin D level too as I have had problems with this before

On the low side, according to an extract from the book, "Could it be B12?" by Sally M. Pacholok:

"We believe that the 'normal' serum B12 threshold needs to be raised from 200 pg/ml to at least 450 pg/ml because deficiencies begin to appear in the cerebrospinal fluid below 550".

"For brain and nervous system health and prevention of disease in older adults, serum B12 levels should be maintained near or above 1000 pg/ml."

Serum folate. Value. 4.49. ng/ml. Normal range. 3.90 - 26.80.

Low. Recommende level is at least half way through range (15.5+ with that range).

You might want to consider a good B Complex (eg Thorne Basic B) which will raise both folate and B12.

Serum ferritin. Value. 17.2 ng/ml Normal range. 14.0 - 186.0

This is dire, virtually bottom of range. You said in our previous post that you are not showing low iron. Ferritin is iron store. Have you had an iron panel done as well? If not you should ask your GP to do one, also full blood count.

Free. T4 level value 26.7 pmol/L normal range 12.0 - 22.0

Serum TSH level Value 0.66 mU/L normal range. 0.27 - 4.20.

You say you take levothyroxine for hypothyroidism. Your over range FT4 suggests you are overmedicated. You need FT3 tested at the same time as FT4 and TSH, this will if you are converting the storage hormone T4 into the active hormone T3.

Also need both TPO and TG thyroid antibodies tested for Hashimoto's (autoimmune thyroid disease) if these antibodies have never been tested

About 90% of all primary hypothyroidism in Uk is due to Hashimoto's.

Hashimoto's affects the gut and leads to low stomach acid and then low vitamin levels

You clearly have low vitamin levels

Low vitamin levels affect Thyroid hormone working

Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten. Dairy is second most common.

According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps, sometimes significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)

Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies

Ideally ask GP for coeliac blood test first or buy test online for under £20, just to rule it out first

Trying gluten free diet for 3-6 months. If no noticeable improvement then reintroduce gluten and see if symptoms get worse
