Hypothyroidism is failure of the thyroid gland (for various reasons).

Hashimoto's is autoimmune thyroid disease and the immune system attacks the thyroid and gradually destroys it, resulting in hypothyroidism. It's said that about 80-90% of hypothyroidism is autoimmune. Hashi's is confirmed by raised antibodies - Thyroid Peroxidase (TPO) and/or Thyroglobulin (Tg).
