Advice please on test results.

Hello, please can you give me advice on these test results. They are limited as taken by go. The result has come back as abnormal.

I have Hashimotos and take 75 mg levothyroxine.

TSH level 0.13 mu/L. (0.2 - 4.0)

Serum free T4 level 21.4 pmol/L (11.0 - 22.6)

I have been feeling unwell for 6 weeks. Usual symptoms that I experience roughly every 6 months. No energy, muscle weakness on activity, feeling exhausted after physical work, anxiety and low mood, body racing, unable to have caffeine as it makes it much worse, a strange feeling at night times like restless legs but throughout my body, I could go on!!

I am not sure what to do next.

You need to get your FT3 tested. It could be that you're a poor converter. Or, it's the Hashi's playing up. You could be coming down from a Hashi's 'hyper' swing.

Thank you for replying. What would cause a Hashi's hyper? I have both under and over active symptoms. As my TSH is low could I be hyperthyroid? I will get a full test completed.

No, you can't be hyperthyroid, because you're hypothyroid. The thyroid doesn't suddenly go from under-producing hormones to over-producing. What happens with a Hashi's 'hyper' swing is that the dying cells dump their stock of hormone into the blood causing FT4/3 levels to suddenly shoot up. But, that's not true hyperthyroidism where the gland is over-active.

What causes these immune system attacks on the thyroid is anyone's guess, really. I don't think anyone really knows.
