Query on absorption problem

Hello, My name is Manasi Anawekar. I am a 46 years old perimenopausal woman with Hashimoto. I take T4 50mcg and T3 60mcg compounded thyroid, but not feeling better at all.

My current blood report is as follows :

T3: 2.66 (range 2.30-4.20)

T4: 0.87 (range 0.89-1.76)

TSH: 1.58 (range 0.35-5.50)

ANTI TPO : >1300.0 ( range <60.00)

Could you please help me find a solution to my problem?

Do I need to try a different kind of T3?

I stay in India. So there is only 1 compounding pharmacy in the whole of India and there is no awareness about T3

You need an increase in dose, with below range t4 and low in range t3 you must feel awful.

If you can I would try levothyroxine plus liothyronine. Your fT3 and fT4 should be much higher on these doses. It could be an absorption problem but much more likely the compounded thyroid is substandard. Compounding pharmacies have been known to make mistakes in the past so I would switch to thyroid hormone tablets if you can.

Can you describe your symptoms as you don't say how your feeling? Cold, gaining weight and slow/ speedy and hot/ weight loss etc - you can be tired either way? If you are feeling the former 50 mcg sounds a tad low so maybe increase that first by 25mcg to see if that helps any - even if you are in middle of the range you have a little room.

How do you take your thyroid hormone? Do you take it all in one go, or do you split the dose? Do you always take your thyroid hormone on an empty stomach and wait at least an hour before eating or drinking anything other than water? Do you take any other supplements or medication at the same time as your thyroid hormone? :)
