After seeking advice I spoke to my GP and he gave me a new prescription. And I now have a different brand of levothyroxine so now I should hopefully start to feel better.

I have taken Actavis for years with no problems, yet it doesn't suit some people. We're all different and have to find what suits us best.
I'm trying to find all the information from the packaging so I can fill out a yellow card . Can't find expiry date. It asks about packaging which is a box I presume?. It appears the teva new formula are manufactured in Croatia. Will persevere with the yellow

Yes, the packaging should mean the box.
I've just looked at my Activis tablets. The expiry date and batch number are on the box but also check the blister pack. With mine, if you look at the foil side of the blister pack then one short end has imprinted the expiry date and the other short end has imprinted the batch number. I imagine your Teva tablets may be the same.
Good that you've got a different brand of Levothyroxine. It might take a few weeks to start to feel better. It takes about 6 weeks for levothyroxine to get into all your cells. If you still have symptoms in a couple of months time, you might need to explore other factors like your vitamin levels.
