Need your expert guidance. T4 Total 4.9 , T3 total 0.77, TSH 0.02 are my recent results. Please guide me. Not taking any medicine.
Well, that's difficult. Your TSH is very low for someone not on thyroid hormone replacement, but your T4 and T3 say that you're hypo.
TT4 and TT3 are not the right tests, though. They tell you very little except that your thyroid isn't making very much hormone - probably because there's not enough hormone to stimulate it.
I'm not medically trained, but it looks to me like you have Central Hypo. CH is when the problem lies with the pituitary (Secondary hypo) or the hypothalamus (tertiary hypo) rather than the thyroid itself (Primary hypo). If there's not enough TSH - which is produced by the pituitary - then the thyroid will not make hormone.
What did your doctor say about your results?
You have extremely high TPO thyroid antibodies
This confirms you have Hashimoto's also called autoimmune thyroid disease
Your TSH is so low this suggests central hypothyroidism or very low vitamin levels (or mix of both perhaps)
You need referral to and endocrinologist who is a pituitary expert
For full Thyroid evaluation you need TSH, FT4, TT4, FT3 plus TPO and TG thyroid antibodies and also very important to test vitamin D, folate, ferritin and B12
Have you had vitamin levels tested?

All thyroid tests should ideally be done as early as possible in morning and fasting. This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, GP will be unaware)

Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten. Dairy is second most common.
According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)
Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies
Ideally ask GP for coeliac blood test first
