Hashimotos and Hypothyroid - Pregnancy

Hi everyone,

I haven’t posted for ages! It’s been such a weird year for me with loss and illness. But I’ve recently found out that we are expecting so it’s better news!

My question is regarding my T4. I’m 9 weeks pregnant currently, I’ve been referred to an endocrinologist by the doctor and my bloods are being done every 4 weeks (only TSH because they don’t test for anything more). I do test via medichecks when I have some spare money, but last time I had a thyroid check plus was last year.

My T4 seems to always stay around the same no matter what my TSH level? Is this relevant or nothing to worry about.. here’s my most recent results.

TSH range mIU/L - 0.27-4.2

T4 range pmol/l - 12.0-22.0

December 2017 (Levothyroxine 50mcg)

TSH - 2.66

T4 - 16.9

May 2018 (Levothyroxine 75mcg)

TSH - 1.67

T4 - 16.4

I was only upped to 100mcg when finding out I am pregnant 9 weeks ago.

July 2018 (Levothyroxine 100mcg)

TSH - 1.14

T4 - 16.5

NICE guidelines state that TSH must be between 0.2 and 2.5 when pregnant, this is the level my

GP is working off. My TSH level was within NICE guidelines when we conceived.

Would there still be any risk to the baby if my thyroid remains within the guideline levels?

I am glad you are pregnant but you will be wary at present as miscarriages can occur. However, this is a link with lots of useful information which will be helpful.

Congratulations. Your TSH looks well within range. They seem to be testing frequently too which is good

What about vitamin levels?

What supplements are you currently taking?

Get your GP to retest vitamin B12, folate, ferritin and Vitamin D if not been tested for a while

Folate and B12 are especially important for the baby

Hello, thanks for you replies :)

I thought I would post again once I had my results back, I only received these yesterday.

I received another set of bloods done for TSH and T4 from the Endo clinic. I fasted for all the bloods I had taken that day.

Thyroid

TSH - 0.32miu/L (range 0.30-5.00)

T4 - 14.1pmol/L (range 11-22)

Iron studies

Serum iron level -

13.9 umol/L

10.0 - 33.0

Serum transferrin -

2.63 g/L

2.50 - 3.80

Unsat iron binding capacity -

66 umol/L

36 - 77

Transferrin saturation index -

21 %

N/A (no range)

Serum ferritin -

26 ug/L

10 - 291

Normal

Serum Folate - 24ug/L (range <5.4ug/L)

Previous folate in November 2017 was 12.2ug/L (abit confused as on this test it says >5.4ug/L is fine)

Serum vitamin B12 - 292ng (range 181-910)

Previous B12 in November 2017 was 338ng so has gone lower.

The doctor insisted that he wouldn't run a Vitamin D test due to me being on 10,000 IU for 6 months and 1,000IU per day since then. He point blank wouldn't run the test.

I am slightly worried about the high folate as I only take 400IU of Folate (not folic acid) per day. This could be because my B12 is lower and isn't helping to convert the folate as it should. But I have read folate is usually only an issue if it is 4 to 5 times higher than the reference range in women. I have seen other NHS reference ranges be anything below 20ug/L is fine, so that would mean I am only slightly higher than normal.

I am now also 12+5 days pregnant and so far so good :) apart from the extreme tiredness and the usual!

Any insight in to the blood results would be good. Doctors haven't even rang me, I have patient access so saw them online. My Kidney creatinine is also abnormal (low) as is Serum UREA level (low).
