You should stop B12 3-4 days before blood draw.
I think that 3-4 days is wholly inadequate. However it makes sense to ask over on healthunlocked.com/pasoc - they know far more than do I.
I have a feeling the answer is months...but I'd noted this - perhaps if you are taking B12 cutting it out for a few days before bloods is proportionately effective!
Yes, if you want a baseline level then about 4 months without supplements is about right I believe. I have found that testing whilst supplementing will tell you if the dose of supplement you are taking is too much, too little, or the right amount to maintain a level. After supplementing daily my level went to around 1400ish, I lowered dose to a couple of days a week and now it's fairly stable at around 900-1000.

You should leave 24 hours between last dose of Levo and blood draw. So if you take your Levo in the morning, you take tomorrow's (Tuesday's) dose of Levo as normal, blood draw Wednesday morning, Wednesday's Levo after the blood draw. If you leave longer than 24 hours your results will show a false low FT4.
