I take Levothyroxine at bedtime around 11pm - if I miss my dose before having an early morning blood draw, will I have any side effects from taking the dose after the test then again at bedtime?

Doubtful, but you will have a false low FT4, because it will be too long between your last dose of levo and the blood draw. It would be better to take the levo on the morning of the day before the blood test. Then go back to taking your levo at night.
