Grapefruit juice may slightly delay the absorption of levothyroxine, but it seems to have only a minor effect on its bioavailability. Accordingly, the clinical relevance of the grapefruit juice-levothyroxine interaction is likely to be small.

Moderation is key! A study investigated the consumption of 200 mL of grapefruit juice three times daily for only two days, and results showed a 10 percent decline in the absorption of levothyroxine due to grapefruit consumption.

Perhaps having one grapefruit every now then will not make a difference if taken a few hours away from thyroxine.

I have always had half of a pink grapefruit for breakfast and continued to do that even when I was taking thyroxine as part of my block and replace treatment.

I used to take my levo very early on when I was in bed then wait for an hour and a half or so and have the grapefruit at breakfast time. I don’t think it made any difference to anything.
