Hi new friends.

I first saw someone about my thyroid over 45 years ago and only once had blood test results that suggested any problem, of all the tf tests. I don't know what that test was but it indicated a possible problem with my pituitary gland. I have over the years had many symptoms but no treatment because of blood results.

Come up to date and the goitre I have been growing over all those years has grown to be a 'multi module goitre which has reduced by trachea to 10mm'. So I have an appointment with ENT next week.

I have discovered that there is an auto immune thyroid condition. I have already got an auto immune condition. No idea if that is relevant.

Comments very welcome re possible treatment, what might be going on and anything else words of advice.

For full evaluation you ideally need TSH, FT4, FT3, TT4, TPO and TG antibodies, plus vitamin D, folate, ferritin and B12 tested

Essential to test antibodies and vitamins

If you have one autoimmune disease, then yes, it's more likely to get another

Over 90% of primary hypothyroidism in Uk is autoimmune

See if you can get full thyroid and vitamin testing from GP. Unlikely to get FT3

Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have money off offers.

All thyroid tests should be done as early as possible in morning and fasting and don't take Levo in the 24 hours prior to test, delay and take straight after. This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, GP will be unaware)

Wow. Thanks SlowDragon. That will take me some time to digest.

I have been referred to ENT by my rheumatologist and already been given an appointment for next week. I will see where I go from there armed with the info you have given me.

I have been on high dose steroids for 2.5 years so that may interfere with immune system tests.

Wondered why they suggested it was pituitary issue? Have you had MRI or pituitary function tests or electrode tests

Autoimmune thyroid disease (Hashimoto's) a very high percentage of us find gluten free diet essential

Hashimoto's affects the gut and leads to low stomach acid and then low vitamin levels

Low vitamin levels affect Thyroid hormone working

Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten

According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)

But don't be surprised that GP or endo never mention gut, gluten or low vitamins. Hashimoto's gut connection is very poorly understood

Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies

Ideally ask GP for coeliac blood test first
