My GP put me on Levo for which I was on for a year but was having pains in my muscles. Approached my GP and asked if I could have a private prescription for NDT
He decided to write to an end and was hoping he would make me an appt to discuss the matter however the endo wrote back saying that I should not be on any meds as he felt my numbers were in range even though I felt dreadful. He wouldn't entertain the NET as he stated that disease was impossible to administer ( which I know is untrue). I decided to come off Levo for 3 month after a discussion with my doctor. My muscles started to recover but after a blood test my thyroid was worse ( obviously ). But i decided to carry on med free whilst doing research hoping to find a med more natural. Muscles back to normal but I was feeling horrendous so had no choice but to go back to Levo as I didn't know which way to go with self medication.
I am now on 25mcg Levo but again having muscular problems and feeling so horrendous.
Bloods were done after endo comments as I felt I needed a broad test
Hope someone can help me as I feel desperate.

You don't need to repost as I enlarged a couple of times and I could read results fine.
Taking your antibodies alone into consideration, you have an Autoimmune Thyroid Disease called Hashimoto's as they are high. This should warrant you to have thyroid hormone replacements, even if your TSH was in range. In the UK, doctors have been told not to diagnose if patient's TSH reaches 10 which is ridiculous.
It is good your doctor has given you 25mcg levothyroxine but you need an increase every six weeks until your TSH is 1 or below.
Going gluten-free can reduce the antibodies which are attacking your thyroid gland until you become hypothyroid. Treatment for hashi's is the same as for hypo.
Your B12 is low so before you supplement with B12 Methylcobalamin sublingual tablets, ask for an intrinsic factor test to exclude pernicious anaemia.

Levothyroxine should be taken, usually first thing, with one full glass of water and wait an hour before eating. Your dose of 25mcg isn't sufficient and the starting dose is 50mcg.

Blood tests have to be at the very earliest, fasting (you can drink water) and allow a gap of 24hours from your last dose of levo and the test and take it afterwards. This helps keep the TSH at its highest as that seems to the only result doctors look at.
Get intrinsic test first as taking supplemental B12 before would mess up results. I have quarterly B12 injections as I have P.A. but I also supplement with B12 methylcobalamin B12 sublingual - from Amazon. Sublingual dissolves under the tongue and due to hypo our digestion may be not so good so sublingual.
