FT4 below range means your friend is hypothyroid and needs Levothyroxine to replace low thyroid hormone. High TSH indicates primary hypothyroidism but low-normal TSH with FT4 below range indicates central hypothyroidism which is usually due to pituitary or hypothalmus dysfunction. If your friend's GP thinks central hypothyroidism is likely she should be referred to endocrinology for investigation in case sex and growth hormones are also deficient.
