A friends thyroid blood test was flagged low. TSH mid range (she couldn't remember)

But T4 was 10. Range 12-22. GP said no need to medicate .

Quite new to this but told her she needed further advice.

She described several familiar symptoms to me.

I felt I needed so much to help her!

Can anyone please comment on such a low level of T4 ??

FT4 below range means your friend is hypothyroid and needs Levothyroxine to replace low thyroid hormone. High TSH indicates primary hypothyroidism but low-normal TSH with FT4 below range indicates central hypothyroidism which is usually due to pituitary or hypothalmus dysfunction. If your friend's GP thinks central hypothyroidism is likely she should be referred to endocrinology for investigation in case sex and growth hormones are also deficient.

Extracts from NICE guidelines:

Secondary or central hypothyroidism is the result of insufficient production of bioactive TSH due to a pituitary or hypothalamic disorder.

Urgent referral to an endocrinologist should be arranged if:

Secondary hypothyroidism is suspected.
