I have an appointment with the nurse practitioner tonight as continue to feel awful. I have just looked again at my TSH results January they were 2.56 now 3.74 ((0.4 - 4.00))? should this be happening when on Levo?

It will happen, if your thyroid function and/or conversion are getting worse. Then again, it could be because you didn't have the tests done at the same time of day.
Is the nurse going to do bloods tonight? If so, you're going to have a very low TSH. The best time to do the tests in early in the morning, after having fasted over-night, and leaving 24 hours between your last dose of levo and the blood draw.
I don't think she will as the GP declined to do them again last week. I had my recent test at 10.00am which was higher than the previous test which was done at about 9am. I requesting to have them done again after finding out I should be omitting medication and having fasting bloods on this forum....the GP totally dismissed this stating that they would never ask anyone to do this! I'm at a loss. I have done a little bit of research regarding what range is "best" and it seems I should be in the lower range. I will be taking this with me but I'm not sure what good it will do!.

You don't need the test repeated. Your TSH is rising because your thyroid function is declining and your current dose is insufficient and needs raising.
The goal of Levothyroxine is to restore the patient to euthyroid status. For most patients that will be when TSH is 0.4 - 1.0 with FT4 in the upper range. FT4 needs to be in the upper range in order that sufficient T3 is converted. Read Treatment Options in thyroiduk.org.uk/tuk/about_... Arrange future blood tests early in the morning when TSH is highest and fast (water only) as TSH drops after eating and drinking. Take Levothyroxine after your blood draw. These are patient-to-patient tips to get the best out of testing. You don't need your GP's permission to follow them.

My Endo, when discharging me back to GP for monitoring (back in 2012) wrote GP that my target TSH is 2. However I have learned from this forum that 1 is ideal. My result in April was 1.94 therefore no action as GP was happy with this.
Yours is even higher on both occassions. So I am assuming that if that had happened to me my GP would have acted upon result. At the same time I had nutrients checked at my request. Vit D and Folate both low and presription given for supplements for both.

Ferritin?

Do you have Hashimoto's?
Your TSH is too high on both occassions. What did Endocrinologist say your target TSH is? What dose Levothyroxine were you put on in January? What dose are you on now?
Have you a started to take any other/new medication at the same time as your Levo or otherwise?

Forget the 'normal' range TSH once diagnosed Hypothyroid. Why not ask Nurse tonight to look back through your notes and see what Endo said your Target maximum TSH should be when you were first diagnosed?
