Yes, folate deficiency. What has your GP said about it? Perhaps he should be prescribing folic acid?

Although your B12 isn't deficient, it's too low. Anything under 500 can cause neurological problems. Recommended level is very top of the range, even 900-1000 (but your GP won't know this unfortunately).
I don't understand why your liver function test is marked satisfactory when they've also marked it with an ! and your serum alkaline phosphatase is over range. It might be an idea to ask your GP about this as well. It makes you wonder why they have ranges if they ignore them!
By the way, your Ferritin is very low as well, it should be half way through it's range.

Yes, folate is deficient. My GP prescribed 5mg folic acid when I was folate deficient. Alternatively you can buy 400-800mcg folic acid or methylfolate without prescription.
B12 is sub optimal. There can be deficiency when it is <500. I would supplement 1,000mcg methylcobalamin plus a B Complex vitamin but wait a couple of weeks after you start supplementing folic acid.
Ferritin 40 is sub optimal. Halfway through range is optimal. You could supplement iron with 1,000mcg vitamin C to aid absorption and minimise constipation.
High alkaline phosphatase may not be significant as your other liver results are normal. labtestsonline.org.uk/under...
I assume you are taking Carbimazole for hyperthyroidism as your FT4 and FT3 have decreased although they remain high. Carbimazole can elevate liver enzymes so your GP should continue to monitor liver when you have your next thyroid function test.
I got 5mg folate from a very well known shopping site. Worked well!
