How to explain symtoms to GP

Originally diagnosed in June. Currently taking 100mcg levo last bloods early November results tsh 0.6 range 0.05 _5.00 t4 10.2 range 10 _22. Dose increased from 75 too 100, gp at time said retest 6 weeks then refused to test till February. I feel swollen all over legs especially. Neck, feet and hands stiff, numbness and pins and needle in hands and feet. Need at least two hours on morning to loosen up, then by tea time wiped out. Have gained 8lbs despite eating very little. Just feel like giving up have suggested private blood tests to gp seemed unwilling to acknowledge results if I got them done. How do I explain to gp I feel like giving up:(
