A TSH of 21 indicates hypothyroidism, because the TSH is "Thyroid Stimulating Hormone".
If you have such a high TSH it means that your pituitary gland is seeing a shortage of thyroid hormone in your blood, and is telling the thyroid to produce more. If the thyroid is not responding properly then the TSH will continue to rise. 21 is very high.
