Yes, as your TSH increases a larger dose of levothyroxine is required to bring it within range.
Have your Thyroid Peroxidase antibodies been tested? Sounds like you may have auto-immune thyroiditis (Hashimoto's) which fluctuates as your thyroid sputters into life and back to useless again.
Yes, but some blood results would be helpful to see what has been happening. If your GP doses by TSH, he may have reduced your dose when not necessary.
That is a huge decrease! Essentially your dose was halved which is much to drastic unless you were experiencing tachycardia of atrial fibrillation or something similar. A drastic decrease like that would normally only be required for a couple of weeks to bring the levels down before increasing again. I think yours might have been left too low for too long.
I agree with the others that you probably need to increase again.

Going by your free t4 would be far more helpful. Is it at the top or just above the lab range?
