I don't know the answer, but I can relate to what you're saying. I tried fasting at one point to see if it would pause my IBS if there was no food in there, and it was fine for a while - I think 24 hours or so - and then I got really bad pains that cleared up as soon as I had some food. Pain is the body trying to tell us something is wrong but it sure doesn't give us enough details!! It's tempting to say if you can get your pains on an empty stomach then they're not caused by food, but the people who write about food intolerance say you can react as much as 24 hours or more after eating something you're intolerant to, so that might not be true. My IBS was not caused by food intolerance, or stress, for what it's worth, but those are the two causes people mention most often for IBS. Also I never used to get normal hunger when my IBS was bad - it was actually quite hard telling when I needed to eat because my gut was just uncomfortable all the time. We are not trained to be very aware of our bodies, quite the opposite, so maybe if we had more animal awareness the pain messages would be more useful and we might respond to them better.
