useful 'Evidence' that TSH between 0.04 - 0.4 has no increased risk to patients on Levothyroxine (UPDATED~new study does show small risk)
This paper forms part of the evidence base for the current NHS thyroid treatment guidelines , it was referenced as evidence to back up their concerns about "low TSH /overtreatment with levo/ risk to bones and heart" ( so they cannot say it is 'not good enough evidence' when you put it under their

Serum Thyrotropin and Triiodothyronine Levels in Levothyroxine-Treated Patients
Comment by Bianco: In LT4-treated patients (blue box=ref. FT4 range), the increase in FT4 is associated with a major drop in TSH, but the increase in blood T3 is minimal. LT4 is really good at normalizing TSH; not nearly as good at restoring T3, the active thyroid hormone. I don't think

Plasma Metabolomics Reveals Systemic Metabolic Alterations of Subclinical and Clinical Hypothyroidism
This paper does what I have long been waiting for. Takes some blood, just the serum, and do a comprehensive analysis of pretty much everything they can detect in that serum. The subjects were divided into Control, Subclinical Hypothyroid and Hypothyroid groups. They found clear differences

Keeping Interferon refrigerated
Hi, I've recently started taking Peginterferon alfa-2a, and I keep it in my fridge because that's what the instructions say. I'm going to visit my girlfriend in Scotland a week tomorrow. My journey is going to take two days, so I'll leave home on Saturday morning, stay in a hotel halfway on Saturday

Interferons as the First Choice of Cytoreduction in ET and PV
»Interferons are a group of cytokines with immunomodulatory properties and were the first immune therapies used in the treatment of cancers, including hematologic malignancies. Use of standard interferon alfa-2b for MPN treatment has been limited by its difficulty in administration and adverse effect

Gluten-free diet and vitamin D uptake in patients with Hashimoto disease
This paper demonstrates that gluten-free diets with Hashimoto's may have an influence on vitamin D uptake. Just a thing to be aware of. Gluten-free diet attenuates the impact of exogenous vitamin D on thyroid autoimmunity in young women with autoimmune thyroiditis: a pilot study October 2022

Yet another definitive paper on relation between FT4 and TSH
This paper brings together my collaborator Prof Hoermann and a similar group in Australia who have been studying the phenomena in parallel with us. It shows that TSH and FT4 are linked, but there is no correct way of measuring sensitivity of TSH. In this it brings our two groups together (not without

Pregnancy and thyroid function
The rate of relevant publication is getting fiercer and fiercer. This August publication discusses the superior measurement of FT4 in pregnancy rather than TSH. The whole paper is available. I append the abstract here. Note the claim of superiority in diagnosis using FT4 over TSH. The application

Hypothyroidism in pregnancy
This paper examines the relationships between TSH and FT4 as diagnostics in pregnancy (behind a paywall ). No FT3 done. Hypothyroid symptoms throughout pregnancy are predominantly associated with thyroxine and not with thyrotropin concentrations August 2022 Thyroid DOI: 10.1089

A recent review of the failings of the currently held model for thyroid treatment
I don't think I've already posted this. This review again sets out the failings of the TSH-T4 dominated model of thyroid therapy and treatment. Open to download Endocrine https://doi.org/10.1007/s12020-021-02851-6 VIEWPOINT Thyroid testing paradigm switch from thyrotropin to thyroidhormones—Future

Thyroid hormone treatment intensity linked to increased risk of cardiovascular mortality
Just how long can this go on? Positively defining in terms of TSH alone. "… exogenous hyperthyroidism or hypothyroidism, which is having thyrotropin levels inappropriately below or above the normal range". And then going to to suggest it is the thyrotropin level which is the cause? Without having even

Tania Smith's take on our recent model paper
Since I'm certain that some of this conversation with Tania Smith will be converted to an article eventually in thyroid patients.ca I thought I would relay verbatim her current thoughts after reading it. When the article from her eventually surfaces I am sure it will spout damning flames about what is

The Starting Dose of Levothyroxine in Primary Hypothyroidism Treatment
This paper compared patients started on a calculated full replacement dose of levothyroxine against starting at 25 micrograms and incrementing every four weeks. Since this paper - and others - the idea of starting on full replacement dose has permeated the guidelines in several countries. Unfortunately

Suboptimal Thyroid Hormone Replacement is Associated with Worse Hospital Outcomes
Not saying this paper doesn't have its place. Of course it does, otherwise there is too much guesswork and assumption. But, I ask you, is anyone actually surprised by this? The cost of treating (in terms of length of stay and/or readmission) must be vastly more than providing adequate levothyroxine.

Peg injections - two in one day
Hi all, I’m on peginterferon Alfa-2a for ET and a few weeks ago my dose was increased to 180mg. The hospital pharmacy deliver my medication and I can only assume they’ve ran out of 180mg syringes as I’ve been sent about 8 boxes of 90mg pre filled syringes meaning I’m expected to do 2 injections after
