Non standard Firmagon dosing effective for 3 months from single injection.
Total
testosterone
level is still castrate at 11 ng/dl. Free T 0.09. PSA continues to drop towards a nadir. (Starting at .25, now at .11, two months post IMRT to pelvis.) It appears Firmagon can be used effectively for ADT at less frequent dosing intervals than standard for ADT.

Why should I continue with ADT ?
I don't know any valid reason why I should continue my
testosterone
zero state any longer.. I became "castrate resistant" after about 3 months on my initial ADT (Firmagon) and since then I have continued having similar injections which seemingly have no reaction to my pca state.

Testosterone- Should I be tested and monitored?
I have asked my MO about testing my
testosterone
but he says not necessary as my numbers look good. Should I insist or is he right not to do it until PSA starts rising? Looking for the collective wisdom of this wonderful community.

has gone up to a normal range, since he is only taking Xtandi. What surprised me is that despite his
testosterone
going up -it's 6- his Psa lowered and also his alkaline phosphatase lowered from 1000 to 500? Any ideas as to what is happening? I hope this good news will continue!

Can Xtandi actually kill advanced prostate cancer?
After 3 months of no ADT drugs, my
testosterone
is at 90 and PSA is still at 0.000 ! The only known mets have been to pelvic LNs that were removed at time of RP. Other than RP and ADT, I have had no other treatments. PSA after surgery was still around 11.00 before starting ADT.

Oligometastatic PCa - There’s Hope!!
The plan is to measure my PSA and
testosterone
every 8-weeks and see how I do. The hope is a durable (long term) remission or a cure. So grateful my oncologist has kept Fighting this with me, treating me as an individual and not just following the standard of care.

/ Free Androgen Index, and upon investigation (MRI) they found a micro adenoma (Pituitary Tumor- benign). I also have primary Lymphedema in my legs, and my lymphatic system is sluggish in legs.

DHT blood test and female pattern hair loss

is low normal. Vit D, B12 zinc are very good as heigh normal. Ovaries are normal and i haven't inherited it. I also have thyroid problems but even when the thyroid is normal and I feel good my hair loss is still very progressive. I'm 47 not menapausal.

Looking Back
In 2020, I expect my PSA to remain low until my
Testosterone
rises from castrate level. If the change follows the same pattern as 9 month of ADT I had before and after Cyberknife, it could take up to nine months. Unexpected things can happen and I may find myself with castrate resistant cancer.

Disease progressing after chemo. What now.
Testosterone
is <3. Xtandi and zytiga have failed. Had chemo with docetaxil and carboplatin this summer, 6 cycles. PSA went from 18 to 6.8. Last chemo was October. PSA now 20 with evidence of progression in lymph nodes and bones. Lungs and liver clear. Need to decide next treatment.
