5ARIs & the PCa Masking issue.
New study that's likely to be misreported [1]. Finasteride & Dutasteride are 5-alpha reductase inhibitors (5-ARIs). They are used to treat benign prostatic hyperplasia (BPH). They do this by inhibiting the aromatization of testosterone [T] to the more potent dihydrotestosterone [DHT]. BPH-related

Elevated LFTs
Tomorrow is my last day of IMRT, with high-dose Brachytherapy scheduled to follow starting next week. But on pre-op labs my LFTs hav risen dramatically over the past 3 months, up 4-7x from normal baseline. I did not have visceral mets on any of several staging scan types. I am asymptomatic as far as

BAT stumped, padding up for second innings
G’day. This is another report on the saga of my BAT treatment. You will find previous reports in my profile but a very brief summary is: In late 2016 my zolodex ADT was failing, I started bicalutamide and a PSMA scan showed small lesions at paracaval and paraaortic nodes. After about a year the bicalutamide

Acute urinary retention post viral meningitis
Hi, I was hospitalized for 2 weeks in February 2019 for Viral Meningitis. I developed urinary retention in the hospital and a Foley catheter was placed to keep my bladder drained. I was discharged with the catheter and it was removed on March 6th (5 weeks after originally placed) but I was still unable

Testosterone.
New German-U.S. paper below [1]. Somewhat off-topic, but I know that a lot of men still regard testosterone [T] as PCa "fuel". It's not surprising. I was born at the start of the Huggins era & I'm astonished that we are still living in it. Huggins basically got his Nobel Prize for discovering that

Thoughts on BAT
G’day comrades. I am here reporting and trying to analyse my experience up to now of Bipolar Androgen Therapy (BAT). I have concluded that it has reversed the resistance that had built up to ADT (gnhr agonist zolodex) +bicalutamide (Casodex). Its about 6 months since I rechallenged the beast with bicalutamide

Why I'm here-
Prostate hurt so bad on a 12-mile run in '84 I had to screech to a halt and lock my butt cheeks 'til it passed. Had to sleep with a pillow between my legs in the '90's betimes and bled out the penis on a run in '09. So on. Had a non-ischemic stroke on a Friday in '15; Saint Elaine (wife) brought a squeeze

Nocturia.
Nocturia - for those who have it. Since my surgery 15 years ago, I regularly visit the bathroom twice each night to void the bladder. Disturbed sleep (& shortened duration of sleep) have sometimes been associated with increased mortality. In a paper published in September, Steve Freedland & team looked

Dutasteride & Abiraterone. The rewrite.
All quotes are from the papers cited in yesterday's confusing: "Dutasteride [Avodart] & Abiraterone [Zytiga]?" 1] "Abiraterone is metabolized in patients to D4A" (Δ4-abiraterone) 2] D4A "has even greater anti-tumor activity" than Abiraterone 3] but "D4A is converted to ... 3-keto-5α-abi" (by 5alpha

Dutasteride [Avodart] & Abiraterone [Zytiga]?
Something I missed - apologies if it has been discussed. Dutasteride "was approved by the FDA for the treatment of BPH in November 2001 and was introduced into the United States market the following year under the brand name Avodart." ... "The patent protection of dutasteride expired in November 2015

Should I do more?
PSA decreased from 1.4 to .355 in one month after going back on ADT after five month holiday . This time trelstar only; stopped casodex and dutasteride. Had sbrt to two bone nets. So I’m still castrate sensitive (T = 9.0) after 4 1/2 years of IADT and SRT to mets discovered on each holiday in 2015,

Males : Is your frequency of urination is high? It could be BPH !
The Dutas T is a combo drug of
Dutasteride
and
Tamsulosin
. The former tries to reduce the size of the enlarged prostate and the later is an Alpha blocker which tries to relax the muscles in the prostate and bladder.
