Trying To Hold Onto Hope
Hi all, I last wrote on here back in the fall because my dad was deteriorating quickly after ending 2nd round of chemo and his
PSA
went to the 300s (see my bio for his medical history).

At NYU
I mentioned a few weeks ago that my brother (the last 1st degree male relative who had not yet had prostate cancer) had had an elevated
PSA
and was seeking care. He had discovered that MSKCC did not want to see him unless and until he had a confirmed diagnosis of prostate cancer.

Radiation for Salvage Treatment
I am about to start salvage treatment after a failed surgery and a rising
PSA
value of 0.1. However, I would like to know if weeks of external radiation with ADT, is the best treatment option here.

PSA Ultrasensitive 0.015 six months after prostatectomy
3 month
PSA
was <0.1, but that was not ultra sensitive. How worried should I be? Next steps? Urologist wants to see what the 9-month
PSA
test looks like, and to get a second Decipher genetic test. (I had one prior to surgery.)

Newly diagnosed
My
psa
in March of this year was 8.2 and then 10.1 four months later, so rising quickly. Biopsy was done at end of August, which showed significant tumor development on the right side and a Gleason score of 4,5 or 5,4 in half of the cores taken.

Docetaxel only partially successful
My
PSA
has risen after falling during infusion #2 - a fall from 32 to 25, now risen to 34. 2. The ALP has gone down - from 380 prior to chemo to 193 after 3 infusions.

Brain metastasis
Has androgen & estrogen receptors and express
PSA
. [2i] ii) PC-3: androgen-insensitive with high metastatic potential. Does not express
PSA
. "PC3 is characteristic of small cell neoendocrine carcinoma". Most PCa bone mets are osteoblastic, but PC-3 is osteolytic.

FYI: from the VA research information on Veterans
The researchers suggested these men could be regularly monitored using a
PSA
blood test and offered surgery to reduce symptoms if they develop.

My brother has advanced prostate cancer
PSA
was 44 and he had some mets in his bones(Pelvic). After that he had triplet therapy(every 3 month hormone therapy+ Docetaxol+ Nubeqa), but unfortunately his
PSA
rising right after he finished his chemotherapy. He had an appointment with his MO and she told him he has not many options.

4k before targeted biopsy?
When my
PSA
went up to 7 my Urologist did a biopsy. No fun. Found a spot that was analyzed as pre-cancerous. 6 months later
PSa
is 11 and wants a directed biopsy. I did the MRI to prepare but life got in the way for 5 months. I found out about a 4k test that might show the presence of cancer.

Now for something completely different
If you are in the earlier group start requesting a
PSA
test (not the most accurate test) but its all we have- at 50. Don't wait. I just hope the anaesthetic doesn't stir up the dragon (I'm Welsh) from its lair.

Started with Dox which reduced my
PSA
from 2200(!) to now around 1200. I’m deep in it. I just had my second pluvocto treatment and hoping for the best. What’s hurting the most is feeling abandoned by my family, my partner, and one of my kids.

Pluvicto Aftereffects SMART syndrome?
He had a hiccup with the supply issues, but
PSA
dropped significantly and his side effects during treatment were perfectly manageable, until .... We were heading to NM via West Texas when he began complaining of bright light and headaches. Then he got suddenly much weaker.

radiation whack-a-mole?
Now after two years of undetectable
PSA
, my numbers are rising. A recent prostate-specific PET scan shows a new hotspot in C3, the cervical vertebrae, and they are proposing another round of radiation. I got two years from the last round of radiation, though with significant side effects.

Adjuvant ADT
Almost immediately
PSA
dropped from 4.2. to less than 0.1 and has stayed that way since. Apart from urinary problems and ED no side-effects. My question: what to expect next now that ADT ends coming thursday (
PSA
; erectile function etc.)? Best wishes to all of you, Henk
