Firmagon

My Father restarted his third round of hormone blockade ON 22.11.18 (Firmagon and Zytiga) He used Casodex on his first round with Firmagon. Second round was Firmagon and Zytiga only. His starting PSA when diagnosed was 433 in February 2014.

My husband's doctor is recommending he go to intermittent ADT after six months on Firmagon, then scans if/when his PSA rises. His PSA is at 0.02 and has been for two months, 0.03 before that, and it was at 24.2 at his first shot in August.

Urologist started with Firmagon, monthly. He suggested Docetaxel - oncologist later agree and chemo treatment started - treatment was tolerated well. Different treatments were introduced: Prednisone was added to the Docetaxel Flutamide was prescribed for 6 months.

They want to start firmagon and Zytiga now, but I have a few concerns. He has Parkinson disease ( stage 3 to 4 ) and I am worried about the side effects . I feel if his testosterone declines, his Parkinson's will get worse.

I am now seeing a medical oncologist and have started monthly firmagon injections and will soon start six courses of chemo. Throughout my whole journey I have never felt unwell, except for the occasional emotional downer.

(Firmagon -then switched to Lupron a few months later.) Treated at Mayo, Rochester under Dr. Kwon's supervision (urologist). Did 6 cycles of Docetaxel. Finished Jan., 2019. No visible tumors showing on the MRI.

Have endured 1 injection (1 mo) of Firmagon and 1 injection (3 mos) of Lupron. Uro recommending 1 more injection of Lupron (3 mos). The side effects are awful but I think I can handle one more round with the end in sight.

Completed 25 IMRT (45 Gy) and brachtherapy "boost" (110Gy, 39 seeds) with ongoing Firmagon/Eligard (5 mos thus far. Btw, hate the hormones.) Most recent PSA 0.1 and testosterone <3. Methinks both numbers are unreliable given continued hormone therapy but who knows?

I was put on Firmagon for 3 months, then moved on to Zoladex, which I have been on since. After several scans (MRI, CAT, Nano-MRI, etc.) no definate Mets. were found. In February I began 7 weeks of radiation therapy, with a total of 28 sessions.

I have switch from an inital 240 ml injection of Firmagon to in 20 Nov to Lupron. Lupron will be my long-term ADT by advice from my doc. After the initial injection of Lupron 19 days ago I can see PSA rise from 87 to 107. My inital PSA was 1030 13 Nov 2018.

His first injection was firmagon on October 4, 2018. Then OC switch him to lupron and xygeva every 3 months on 11/6/18. Oncologist said every 3 months will work for him instead of monthly.

Treatment - Firmagon on Oct 19/2018 Bone scan - Nov 5/2018 Showed extensive mets from head to knees. Every single rib. Too many mets to count. More extensive than CT showed. Not much more detail.

I never had any testosterone blood test before Firmagon and Lupron shots as part of cyberknife treatment for an extra capsular recurrence in 2016. In January 2018, when my PSA was undetectable I had T value of 44. Then when my PSA started rising I recovered T.

In 2016, I had three months of Firmagon followed by six months of Lupron as part of cyberknife treatment of my prostate cancer recurrence. Within four or five days of first Firmagon shot I started having hot flushes. My PSA dropped from 1.8 to 0.2 after the second Firmagon shot.

So, I started my ADT (Firmagon) and had "an average response". In the first month, my PSA went from ~100 to 3.14. After 6 months it was at ~0.45. The scans showed the tumour had shrunk some, but the bone lesion was unchanged. Not bad news, but it was not convincing for him.

The urologist suggested Lupron or Firmagon. I opted for Lupron. I also am on Casodex. My MO wants to put me on Zytiga with Prednisone. I like the Casodex and Lupron combo because I am tolerating them well. I am not so sure how I will tolerate Zytiga.
