Just got approved for
Besremi
!! So happy! Took 2 doses of Pegasys and I finally got approved on my 3rd appeal! What are the most common side effects that I might expect to see? I have Budd Chiari syndrome so hoping it’s not too harsh on my liver! Thanks in advance for any advice! Jeanie

My latest CBCs etc on Besremi
A bunch of us are posting our latest. Here's my summary. -Past 5 Bes doses were 75,75,100,100,120. -All the counts we watch are doing well. HCT= 43.7, down from 44.4 prior. This is likely from the dose increase from 75. PLT holding at 288, Neut slightly up at 2.5, in its normal fluctuations.

Besremi
My Hematologist is talking about me considering
Besremi
. Thus far I have not been on any meds. He believes I maybe be a good candidate as I have no had any MPN events. I like the fact it does target the JAK2 gene and I am game to try it.

BESREMI ACHIEVES PATIENT-SPECIFIC TREATMENT GOALS IN POLYCYTHEMIA VERA: FINAL RESULTS FROM THE PROUD-PV/CONTINUATION-PV STUDIES
Depletion of the JAK2V617F alle burden, which may lower the risk of progression to myelofibrosis, was observed in
Besremi
treated patients; JAK2V617F allele burden <1% at 6 years was achieved in 19/92 (20.7%) patients in the
Besremi
arm with baseline allele burden >10%.

A follow up to my post about switching from hydroxyurea to interferon. Questions about switching to besremi or anagrelide.
My hematologist is willing to prescribe whatever I want him to, but he wanted me to look into
besremi
and also anagrelide. I don’t think he has worked with interferon for quite some time and considered it an older medication.

1st Besremi shot
1st
Besremi
shot under my belt- literally! 😀 You all know I was a nervous wreck. Not about a needle- about the medicine. I thought I was going to have a panic attack but it’s done. The process was fine just had to deal with my crazy self.

Update 6.7
It seems that 150mcg is the max dose of
Besremi
that I can tolerate. Fortunately the itching I was getting has not recurred since I have been on daily Claritin. We did confirm that the incident of arthralgia I had in my knee was most likely a
Besremi
AE.

ET diagnosed at 85 yrs. Failed HU because of side effects but platelets down to 555. Considering age and quality of life issues..........
The Hematologist is looking into our insurance coverage for
Besremi
or Pegasys. He tried the HU a second time after problems last week just to make sure it was really from the meds.

Besremi and Phlebotomy
Hello, I had my first PB last week since starting
Besremi
(January 2022).
Besremi
is doing great job on my WBC and PLT, but the HCT has climbed above my goal of 42. I waited two extra weeks hoping a higher dose at 110 might knock it down a bit, but no luck.

Comparing two INFs
This is mostly for science nerds, a comparison of the old PEG-Intron (no longer available) to Pegasys, shedding some light on what the
Besremi
guys are doing with longer dosing intervals. --- A 2004 report in context of hepatitis therapy: https://onlinelibrary.wiley.com/doi/full/10.1111/j.1365-2036.2004.02170

Flu like symptoms
So… on
Besremi
- can have flu like symptoms. Does this mean you just feel like you are sick or are you actually sick and how can you tell the difference? It lowers our immune system, right? Are we contagious when we feel bad? I don’t see how you could ever know for sure.

Questions about AEs on Interferons w.r.t. liver enzymes
I’ve also seen general comments that
Besremi
is “better tolerated” than Pegasys but does “better tolerated” include fewer AEs of the liver enzymes? For Peg and
Besremi
, are elevated liver enzymes one of those symptoms that tend to revert to normal over time as patients get used to the Peg?

3 Peg injections later
I know
Besremi
is supposed to have less side effects, does anyone know if that would be true of this type of fatigue? Thanks!!!

Besremi decision time
Getting to the point where I need to make a decision whether to start
Besremi
. Just had a CBC yesterday & my Platelets continue to rise. I used to be in the middle 500's & yesterday was 660 (after recent 603, 634, 648, 645).

I see a lot of references to
Besremi
. Is
Besremi
also an interferon medication? In Australia we only have access to hydroxyurea or Pegasys.

One Dr's comments on Besremi approval
Encouraging words: <<Pemmaraju noted that the long-term findings demonstrate that
Besremi
is very safe and well tolerated. “I think the strength of this data set is that you have such a long follow-up,” he said. “It’s very comforting to see that.

Update 6.5
I have also experienced some itching with the
Besremi
(Pegasys too). Fortunately that has been well controlled with Claritin. If it appears I will not tolerate the 150mcg dose, the MPN-doc wants me to drop back to 125mcg and try that. He does feel the
Besremi
adverse effects are dose related.
