8/4/23 Update questions for MO
To review, after 19 months on Lupron, Zytiga, prednisone, PSA increased on 1/23 to 0.253, then 0.309 on 2/21. Plarify scan showed only one active site in hip which was radiated. 4/4 PSA dropped to 0.24 which indicated the lesion was knocked out and I would be hormone sensitive and stay on Zytiga. Yea

Senescent neutrophils and their subsequent impact on tumor progression and therapy resistance
I will comment after studying this for awhile, but it does open up an additional treatment path(s?) for PCa. (Excerpts) Calcinotto's team discovered that prostate tumor cells produce a molecule, ApoE, which can bind to and influence a type of immune cells, neutrophils, resulting in these cells acquiring

T is going down.
I’m in my 10 mo. of holiday from Zytiga. My psa has risen from <0.04 to 0.90, my T value has risen from <7 to 24(LO) in 6 mo. Ago and than back down to 20 (LO) as of last week. My MO is concerned about the drop, should I? What is going on this. I feel great I feel strong.

Apology Re: Bring PSA from 6.7 to 3.1 in 4 weeks by pure water fast (3wks) and vegan diet (1 wk).
Dear colleagues, I wish to apologise for using the "cure" in the title of the post I made here about a week ago. For reference the title of the post has now been changed to "Bring PSA from 6.7 to 3.1 in 4 weeks by pure water fast (3wks) and vegan diet (1 wk)." I now realise that I made a big mistake

Pluvicto
Recently had my third dose of Pluvicto, and the numbers are looking good. My PSA fell from about 90 to 35 after the first dose and to about 15 after the second. What they measure in the blood work has either been within the normal range or showing a positive trend. The scans show no new mets, a visible

Imaging Guidelines - Salvage IMRT Planning
I'm planning to start my post-op salvage treatment after my PSA breached 0.1 few weeks ago. I'm OK with the extent of the treatment proposed, which is 5 weeks of IMRT (VMAT) to the prostatic fossa and pelvic LNs. However, I'm not confident with the accuracy of radiation delivery to the target area. Knowing

Any ideas for hot flashes? Getting worse.
Dx in 8/2020 with Gleason 8. Had RP 11/2020. One lymph node involved so MO/RO at MGH recommended aggressive treatment. Went on Lupron and Abi for 2 years and 39 radiation treatments. All drug treatment ended in April of this year. Hot flashed started after a few months on Lupron and were bearable

radiation decisions and second and third opinions
My dad was recently diagnosed with prostate cancer. It has metastasized to two pelvic lymph nodes and a lymph node in his back they initially called “suspicious” because it’s so small if it wasn’t for the PSMA scan it wouldn’t have been picked up at all. He got his first Lupron shot 3 weeks ago and

Was Chemo "Successful"?
I just had my first PSA test after waiting to allow the results to mean something, after 10 rounds of doxytaxel. I was supposed to wait three months but for convenience pushed it by a week. I was hoping for a close to 0 PSA but that is not the case. I see my oncologist in a little more than a week

PSA rise but scans are clear. Appetite and activity level are 0
I have spoken on here a few times about my dad who is on Erleada + ADT but it has failed. They have done quite a few scans recently because he is in significant pain. But all scans have come back “clear” in that there is no significant change in metastasis etc. we are at a point now where he barely

Vacation from ADT
I searched but only came up with vacations at two years or so. I had a meeting with my MO today. He is conservative but suggested I could stop the meds. I was complaining of brain fog. He said to stop the Abi for about three weeks to see if there is any improvement and if not, stop Lupron. My story

enzalutamide dosing
I have ealrly stage 4 prostate cancer, have been on lupron for just over 4 months, and enzalutamide Xtandi for just under 4 months. Not enjoying the side effects of 160 mg I reduced my dosage to 80 mg and tolorate it a little better. Every medial professional and non medical professional cautions me

Androgen Metabolism and Response in Prostate Cancer Anti-Androgen Therapy Resistance, Int J Mol Sci. 2022 Nov; 23(21): 13521
This is by far the best overview paper I have seen on the topic of AR metabolism and the nature of treatment resistance for mainstream SOC drugs currently in use. Recommended reading for those wanting to more fully understand their disease. Here is the Abstract: [i]

ADT and bone health
I am 79 in exceptional physical condition. I have been on ADT after radical Prostatectomy for over 14 months and to date aside from the sexual libido loss I have had very little side effects. I have always been a health / exercise buff & continue to exercise everyday to maintain my mental & physical

Full Pelvic Radiation
My doc stopped the orgovyx I was taking because after just 2 weeks my PSA started to rise 10 points, then the second month it had risen another 10 points.. So now I just got my first Lupron injection. I chose once a month injections to see how well it works. If it works I'll increase it to 3 month injections

Genomic testing
Who benefits from genetic testing and when? Is it mainly when someone becomes castrate resistant and or metastatic that they recommend it? Or is there value in doing it ahead of things while one is castrate sensitive? Had RP and then salvage radiation with 6 months ADT. 1st lab after.. T of 25 and undectable

looking for reassurance or advice
Hello warriors, After exactly 2 years of happily dropping PSA numbers on triplet therapy, my husbands PSA went up this month from .91. To .94 I know MO will say to wait for 3 months of rising PSA before we should panic, but that’s easy for him to say… looking for reassurance as we wait for next months
