i have tested positive for my ANA with Homogeneous pattern, but my recent blood work showed that my ANA was negative but my Anti-DSDNA was elevated to the indeterminate level to 6. My RF came back negative. I get stiffness in my hands, really bad back pain, knee pain, and at times my kneed give out making it impossible to workout without experiencing pain. I do have severe GERD and I do take pantrapozole. I have a history of high levels of thyroid peroxidase, but my TS3 and TS4 have been normal. I have a family history of RA, Raynauds, and Scleroderma. Any thoughts of what this might be?

i also should add that 5 of my fingers have Swan Neck deformities with no recollection of trauma

It does sound like you might have something 'rumbling', but l am not sure there is enough there for your GP to refer you just yet, nor for a rheumatologist to make a diagnosis. Be prepared for a long 'wait and see' method of diagnosis. Unfortunately, there are approximately 6 different connective diseases it could be (might prove to be none), and the experts like to get it right so that you get the correct treatment plan. This could involve several hospital visits over a few years before a diagnosis is even made, or no referral until you develop a high titre positive ANA and a positive ENA, as well as symptoms that point towards ssc, lupus, RA etc such as raynaurds (usually the first unwanted visitor to make an appearance).
