I currently take Pramipexole 0.25 at 7 pm and 0.50 at 9pm. and gabapentin 300mg at both 7&9pm. This seems to work ok most nights. Sleeping by 9:30-10 and alway wake at 2am, go back to sleep till 4-4:30 with leg movement starting to activate. Recently have added 1300 mg of Tylenol at 9pm on 3 nights and on all 3 have gotten a much more peaceful night of sleep and sleep straight through to 5-7 am.

1000 mg is the maximum dose. Taking 1300 mg in one dose every day can increase the risk of liver damage.

I'm wondering why you needed to be adding Tylenol to your already existing medication?

Was it because the old regime wasn't working as well as it used to, and you needed to strengthen it a bit?

If so, then please research "Augmentation"

Yes, and thanks i will be researching augmentation. Interesting that my neurologist prescribed a total of .75 Pramipexole from the start which i am learning is over the max dose. Especially as i am a kidney transplant patient albeit my kidney function in maintaining the 60-70 Gfr range. Thanks

I won't give you my usual rant on medical malfeasance. Aren't you the lucky one?

Well --- actually ---- you will need all your lucky charms set up in a row to get through what you are going to have to.

Having read up on it, you know you have to get off the bl**dy Pramipexol.

Do it as slowly as possible and by as small increments as possible.

Read most any of Sue/ Jools answers to many people in your exact situation.

Don't bother reading my rants.😤😤😤

Follow SueJohnson advice.

There's not much point adding meds to your current regime if you're still on such high doses of Pramipexole.

Your RLS will remain severe unless and until you get off Pramipexole.

Thanks, everything i have been reading appears to point to Pregabalin and Gabapentin as the two primary RX drugs to treat RLS before opioids. Am I interpreting correctly? I will also study the Mayo Clinics new paper on RLS treatment to better educate myself and look for a new Neurologist. Thanks to all who have helped me so far.

Yes. You usually have to try gabapentinoids to see if they work before opioids are prescribed.

I have been taking Pramipexole since 10/08/24 prescribed .75mg daily. After joining forum and doing other research I see that I need to get off pramipexole for augmentation reasons. I started reducing dose about 3 weeks ago and have gotten down to .25mg per evening. I am also taking Gabapentin 300mg twice a day. I have had some good night's sleep and some bad night at the daily dose of 600mg Gabapentin and .25 mg of Pramipexole. I tried no Pramipexole a couple nights ago and had difficult night. I have also been prescribed Pregabalin .25mg twice a day, have not started taking yet as I don't know if I need to be off Pramipexole first? My Physician's assistant is useless, and I am looking for new doc / neurologist. Question is how long I should expect to get off last .25 Pramipexole and can I take Pregabalin now as I am weening off Pramipexole? Any help appreciated.
